There is a persistent and recurrent itching sensation from the throat and Adam's apple to the central depression of the clavicle?
Hello Doctor, since February of this year, I have been experiencing frequent itching in my throat.
When the itching is severe, it can trigger a feeling of nausea, but I do not cough or produce phlegm; it is purely an itching sensation.
Pressing on the muscles above the central notch of my clavicle causes soreness, which also makes the itching more pronounced and may induce coughing.
Additionally, I sometimes feel tenderness on the right side of the central notch of my ribs and some of the ribs, but this pain fluctuates and is not persistent.
In January, I underwent an otolaryngology endoscopy due to a persistent sensation of something stuck in my throat, and the doctor diagnosed me with chronic pharyngitis.
I also have a history of allergic rhinitis, and there was slight edema in the larynx.
Both neck ultrasound and thyroid ultrasound were normal, and treatment has not significantly improved the itching and sensation of something stuck in my throat.
I would like to ask the doctor if the itching in my throat caused by chronic pharyngitis can extend to the area around the central notch of the clavicle? (Previously, I only experienced the sensation of something stuck when swallowing.) Has that area already reached the trachea? What could be the possible causes? Are there any further tests that I should undergo? I am a bit worried that it could be due to a more serious condition.

Hello: The central depression of the clavicle tends to elicit a similar discomfort response upon palpation.
There is no need to use palpation to test for underlying issues.
If symptoms do not improve, consider seeking treatment from either Western or Traditional Chinese Medicine practitioners.

Chronic throat itching can be a perplexing and uncomfortable symptom, and it often raises concerns about underlying conditions. Based on your description, it seems that you have been experiencing persistent throat itching since February, accompanied by sensations of discomfort in the collarbone area and occasional pain in the rib cage. You also have a history of chronic pharyngitis and allergic rhinitis, which can contribute to throat irritation.
Chronic pharyngitis is characterized by inflammation of the pharynx, which can lead to symptoms such as throat itching, discomfort, and a sensation of something being stuck in the throat (often referred to as "globus sensation"). The itching you are experiencing may be a result of this inflammation, which can cause irritation of the throat tissues. Additionally, the swelling noted in your larynx during the endoscopy may also contribute to these sensations.
The discomfort in the collarbone area and the rib cage could be related to referred pain from the throat or neck. The throat and neck are interconnected, and inflammation or irritation in one area can sometimes lead to sensations in another. For instance, the muscles and nerves in the neck can become tense or irritated due to chronic throat issues, leading to pain or discomfort in the surrounding areas, including the collarbone and ribs.
Regarding your concern about whether the irritation has extended into the trachea (the windpipe), it is important to note that chronic throat conditions can sometimes lead to inflammation that affects the upper respiratory tract. However, the absence of cough or phlegm suggests that the lower respiratory tract (including the trachea and lungs) may not be significantly affected at this time. If there were significant involvement of the trachea, you would likely experience additional symptoms such as a persistent cough, wheezing, or difficulty breathing.
As for the potential causes of your symptoms, several factors could be at play:
1. Allergic Reactions: Given your history of allergic rhinitis, allergens such as pollen, dust, or pet dander could be contributing to your throat irritation.
2. Gastroesophageal Reflux Disease (GERD): Acid reflux can lead to throat irritation and chronic cough, which may not always present with typical symptoms like heartburn.
3. Environmental Irritants: Exposure to smoke, pollution, or strong odors can exacerbate throat irritation.
4. Infections: Chronic infections or post-viral syndromes can lead to prolonged throat symptoms.
5. Other Medical Conditions: While less common, conditions such as thyroid disorders or even malignancies could potentially cause similar symptoms, although your recent ultrasound results were normal.
To address your concerns and further evaluate your symptoms, it would be prudent to consider the following steps:
1. Follow-Up with an ENT Specialist: Since you have a history of chronic pharyngitis, a follow-up appointment with an ear, nose, and throat (ENT) specialist may be beneficial. They can perform a thorough examination and possibly recommend additional imaging or tests if necessary.
2. Allergy Testing: If allergies are suspected, allergy testing can help identify specific triggers that may be contributing to your symptoms.
3. Gastroenterology Consultation: If GERD is a concern, a consultation with a gastroenterologist may be warranted to evaluate for reflux and discuss potential treatment options.
4. Symptomatic Treatment: In the meantime, consider using saline throat sprays, humidifiers, or over-the-counter antihistamines to alleviate itching and discomfort.
5. Monitoring Symptoms: Keep track of any changes in your symptoms, including the onset of new symptoms such as cough, difficulty swallowing, or changes in voice, and report these to your healthcare provider.
While it is natural to worry about serious conditions, many cases of chronic throat irritation are related to benign causes. However, staying proactive about your health and seeking appropriate evaluations will help ensure that any underlying issues are addressed promptly.
