Root canal treatment

Hello, two years ago I had a tooth in the upper left back that cracked and I went to see a dentist.
After root canal treatment, a pus-filled abscess developed above the tooth.
Later, swelling and pain extended from the upper left tooth to my left cheek and eye socket, and I started experiencing a persistent high fever.
I went to the emergency room at National Taiwan University Hospital twice for blood tests, but no cause was found.
Eventually, the fever subsided, but I still feel a numbness in the left side of my face, similar to facial nerve damage.
Can root canal treatment cause facial nerve damage? To this day, my left face still feels the same; I can't smile naturally, as if I've had Botox.
If I have the tooth extracted, will my facial condition improve? Recently, the same tooth has been experiencing severe pain and swelling again, and the doctor mentioned that I would need an implant for this tooth.
I would like to know if an implant is absolutely necessary.
Is it possible to have a single crown instead? Or is it true that a cracked tooth cannot support a crown? Thank you.
🙏

Dear Ms.
MEI,
Hello!
1.
The presence of an abscess above the tooth after root canal treatment indicates that the root canal may need to be redone.
2.
Swelling from the left cheek to the eye socket suggests an infection.
3.
There are many causes for facial nerve damage; it is advisable to consult a neurologist for diagnosis and treatment.
4.
(Regarding the same tooth that is experiencing severe pain and swelling, the doctor mentioned that it will require an implant.
I would like to inquire if an implant is absolutely necessary, or if a single crown can be placed instead.) If the root canal treatment is successful, it is possible to place just a single crown!
Take care! Wishing you peace and smooth recovery!
Sincerely,
Dr.

Understanding the complications of root canal treatment and the subsequent dental solutions is crucial for anyone experiencing dental issues, especially after a procedure like root canal therapy. Your situation, involving swelling, pain, and potential nerve damage, raises several important points that need to be addressed.

Firstly, root canal treatment is designed to remove infected or damaged pulp from inside the tooth, which can alleviate pain and save the tooth. However, complications can arise, such as the formation of an abscess (the "膿包" you mentioned), which can lead to swelling and pain in surrounding areas, including the face and even the eyes. This is often due to the infection spreading beyond the tooth itself. In some cases, if the infection is severe, it can cause systemic symptoms like fever, as you experienced.

Regarding your concern about facial nerve damage, while root canal treatment itself is generally safe, there is a small risk of nerve injury, particularly if the tooth is located near the facial nerve. This can lead to symptoms such as numbness or altered sensation in the face, which you described. If the nerve is indeed affected, it may take time for it to heal, and in some cases, the damage could be permanent.
If you are considering tooth extraction, it is important to understand that removing the tooth may not necessarily restore normal function or sensation to your face. The nerve damage may persist regardless of whether the tooth is extracted. However, extraction can eliminate the source of infection and pain, which might provide some relief.

As for your question about dental restoration options after extraction, dental implants are often recommended as they provide a stable and long-lasting solution. However, if you prefer not to undergo implant surgery, a dental crown (牙套) can be placed on a remaining tooth structure if it is viable. If the tooth is cracked or severely damaged, it may not be suitable for a crown, and extraction may be the best option.

In your case, since you mentioned that the tooth is causing severe pain and swelling again, it is crucial to follow your dentist's advice. If they recommend extraction and subsequent implant placement, it is likely because they believe this is the best way to ensure your overall dental health and alleviate your symptoms.

In summary, while root canal treatment can save a tooth, complications can occur, leading to pain and swelling. If nerve damage is suspected, it is essential to discuss this with your dentist or an oral surgeon. Extraction may be necessary, but it is important to weigh the options of implants versus crowns based on the condition of the remaining tooth structure. Always communicate openly with your dental care provider about your concerns and preferences to find the best solution for your situation.
