Glans penis abrasion?

I may have caused this issue by doing DIY for four consecutive days.
Initially, there was just a small red area on the glans, but after seeing a doctor, I was told that antibiotics would suffice.
Now, there is a large area of broken skin on the corona, and it seems to be quite deep.
After consulting a dermatologist, I was prescribed medication and betamethasone to take and apply.
It’s now the second day, and the smaller broken skin areas have improved significantly, but the central, most severe area is still very red and painful.
After applying the ointment to keep it dry, the ointment hardens, and when I wake up the next day, the wound oozes, making the ointment sticky.
After removing the ointment, I can see that the wound is still red and painful without any signs of healing.
I would like to ask the doctor: 1.
How long will it take to heal? 2.
I read online that moist wounds heal faster, but every urologist I consulted said to keep it dry.
Which is correct? 3.
How can I prevent this from happening again? I am already in so much pain that I am afraid.
Thank you for your answers.

Understanding and treating glans skin tears can be quite challenging, especially when it comes to managing pain and ensuring proper healing. From a urology perspective, it is essential to address the underlying causes of the injury, the treatment options available, and the preventive measures that can be taken to avoid future occurrences.

Firstly, it is important to understand that the glans penis is a sensitive area with delicate skin. Tears or abrasions can occur due to various reasons, including friction from sexual activity, improper hygiene, or even aggressive cleaning methods. In your case, it seems that DIY activities may have contributed to the initial injury, which then worsened over time.

Regarding your specific questions:
1. Healing Time: The healing time for skin tears on the glans can vary significantly depending on the severity of the injury and the treatment applied. Generally, minor abrasions may heal within a few days to a week, while deeper tears could take longer, possibly up to two weeks or more. It is crucial to follow your healthcare provider's advice and continue using the prescribed medications, as they can help reduce inflammation and promote healing. If you notice no improvement after a week or if the pain worsens, it would be wise to consult your doctor again.

2. Moist vs. Dry Healing: The debate between keeping a wound moist versus dry is ongoing in the medical community. Traditionally, many healthcare providers recommended keeping wounds dry to prevent infection. However, recent studies suggest that moist environments can promote faster healing by preventing the formation of scabs, which can impede the healing process. For glans skin tears, a balance is necessary. While it is essential to keep the area clean and dry to prevent infection, using a suitable topical ointment can help maintain moisture without creating a wet environment that could lead to maceration. It is advisable to follow your doctor's specific instructions regarding wound care, as they will tailor their advice based on your individual situation.

3. Preventing Future Tears: To avoid future skin tears on the glans, consider the following preventive measures:
- Gentle Hygiene: Use mild, unscented soaps and avoid aggressive scrubbing. Ensure that the area is clean but not overly dry.

- Lubrication: If friction is a concern, especially during sexual activity, consider using a water-based lubricant to reduce irritation.

- Avoid DIY Procedures: Be cautious with any DIY activities that may involve the genital area. If you are unsure about a procedure, consult a healthcare professional.

- Regular Check-ups: If you have a history of skin issues in this area, regular check-ups with a urologist can help monitor any changes and provide timely interventions.

In conclusion, while glans skin tears can be painful and concerning, with proper care and attention, they can heal effectively. It is essential to follow your healthcare provider's recommendations closely and to communicate any changes in your symptoms. If you have further questions or concerns, do not hesitate to reach out to your healthcare provider for personalized advice. Remember, taking care of your skin and being mindful of potential irritants can significantly reduce the risk of future injuries.
