Chest Bone Issues: Symptoms and Concerns Explained - Internal Medicine

Chest bone issues

Hello, doctor.
I have allergic rhinitis, and when I have a lot of nasal discharge, I tend to cough.
While carrying my side backpack, I felt that the angle was just right for my cough, and I sensed that the strap of my backpack was pressing against my chest.
Although I am not experiencing any pain now, I would like to ask if this could accidentally lead to a pneumothorax or if my bones could become brittle.
Recently, a friend was hospitalized because he seemed to have sung too loudly and ended up with a broken rib.
Could you please tell me what symptoms I should look out for, or should I not worry too much? I feel like my situation is somewhat similar to being pressed by a seatbelt.
Thank you!

Xin, 10~19 year old female. Ask Date: 2020/07/02

Dr. Hong Yuanbin reply Internal Medicine

Under normal circumstances, pneumothorax or brittle bones are not common.
Pneumothorax typically occurs due to individual predispositions, while fractures are associated with osteopenia combined with excessive force on the thoracic cage.
Based on your description, pneumothorax or brittle bones should not occur.
Just be cautious not to exert sudden force on the thoracic cage, such as through forceful sneezing or coughing, and there should be no issues.
There should be no direct relation to carrying a bag or wearing a seatbelt.
Thank you for your inquiry.

Hello! Thank you for your question regarding chest bone issues and your concerns about potential injuries. It's understandable to feel anxious, especially when you hear about others experiencing serious conditions like rib fractures or pneumothorax (collapsed lung). Let's break down your situation and address your concerns.

From what you've described, it seems that you experienced some discomfort in your chest area while coughing, particularly when your bag's strap pressed against your chest. It's important to note that while this sensation can be alarming, it does not necessarily indicate a serious injury. Rib fractures typically occur due to significant trauma, such as a fall, a car accident, or a direct blow to the chest. In your case, the pressure from a bag strap, even during a cough, is unlikely to cause a fracture unless there was a pre-existing weakness in the bone.

Rib fractures can present with several symptoms, including:
1. Localized Pain: This is often sharp and worsens with movement, deep breathing, coughing, or laughing.

2. Swelling or Bruising: You may notice visible swelling or bruising over the area of the fracture.

3. Difficulty Breathing: In some cases, pain can make it hard to take deep breaths, leading to shallow breathing.

4. Crepitus: A crackling sensation may be felt if the fracture is severe.

As for pneumothorax, this condition occurs when air leaks into the space between the lung and chest wall, which can happen due to trauma or spontaneously. Symptoms of pneumothorax include:
1. Sudden Chest Pain: This pain may be sharp and can be felt on one side of the chest.

2. Shortness of Breath: You may feel like you can't catch your breath or that your breathing is labored.

3. Rapid Breathing: An increased respiratory rate can occur as your body attempts to compensate for reduced lung capacity.

Given that you are not currently experiencing pain or discomfort, it is unlikely that you have sustained a serious injury. However, if you start to notice any of the symptoms mentioned above, it would be wise to consult a healthcare professional for a thorough evaluation.

In terms of your concern about your ribs becoming "brittle," it's important to maintain good bone health through a balanced diet rich in calcium and vitamin D, regular weight-bearing exercises, and avoiding smoking. If you have any underlying conditions that affect bone density, such as osteoporosis, it's crucial to manage those with the help of a healthcare provider.

If you are still feeling anxious about your symptoms or if they persist, consider scheduling an appointment with your doctor. They may recommend imaging studies, such as an X-ray, to rule out any fractures or other issues.
In summary, while your concerns are valid, the likelihood of a serious injury from the situation you described is low. Keep an eye on your symptoms, and if anything changes or worsens, do not hesitate to seek medical attention. Stay healthy and take care!
