Dealing with Large Warts: Treatment Options and Costs in Taiwan - Dermatology

The cauliflower lesion is too large?

Hello, doctor! A few days ago, I felt discomfort around my anus and discovered a large wart and several fleshy growths using a mirror.
After going to the hospital for examination, I was diagnosed with condyloma acuminatum.
The doctor informed me that one of the warts is quite large (about the size of a ten NT dollar coin) and recommended applying a medication (Podophyllotoxin) for a month before reassessing for treatment.
They also suggested that I seek treatment at other hospitals during this time, but most dermatology departments I visited required a referral.
I feel really upset about being denied treatment...
1.
Are there any hospitals in Hsinchu County or City that can treat such large condylomas? (Which department should I consult?) 2.
What are the typical treatment options for large condylomas, and what is the approximate cost?

Ailan, 30~39 year old female. Ask Date: 2020/07/14

Dr. Wu Yuxin reply Dermatology

1.
Referral to colorectal surgery.
2.
Electrocautery, covered by health insurance.

Dealing with large warts, particularly those diagnosed as condyloma acuminatum (commonly known as genital warts), can be a challenging experience, both physically and emotionally. In Taiwan, there are several treatment options available for large warts, and understanding these options, along with their associated costs, can help you make informed decisions regarding your healthcare.

1. Treatment Options:
- Topical Treatments: As you mentioned, your doctor has prescribed a topical medication called Podophyllotoxin (often referred to by its brand name, Condylox or similar). This medication is applied directly to the wart and works by destroying the wart tissue. It typically requires daily application for several weeks. While effective, it may not be sufficient for larger warts.

- Cryotherapy: This is a common treatment for warts where liquid nitrogen is applied to freeze the wart, causing it to fall off. This method is effective for many patients, but multiple sessions may be required, especially for larger warts.

- Electrosurgery: This technique uses high-frequency electrical currents to destroy the wart tissue. It is particularly useful for larger warts and can be performed in an outpatient setting.

- Laser Therapy: For very large or resistant warts, laser treatment may be recommended. This method uses focused light to vaporize the wart tissue and is effective but can be more expensive.

- Surgical Excision: In some cases, especially when the wart is particularly large or has not responded to other treatments, surgical removal may be necessary. This is usually done under local anesthesia.

2. Cost Considerations:
- The costs for these treatments can vary significantly based on the method used and the healthcare facility. For instance, topical treatments may cost less than NT$1,000 for a month’s supply, while cryotherapy sessions can range from NT$1,500 to NT$3,000 per session. Electrosurgery and laser treatments can be more expensive, often ranging from NT$5,000 to NT$15,000 per session, depending on the complexity and size of the wart.

- Surgical excision can also vary widely in cost, typically ranging from NT$10,000 to NT$30,000 or more, depending on the facility and the extent of the procedure.

3. Where to Seek Treatment:
- In Hsinchu City and County, you can seek treatment at dermatology clinics or hospitals that have a dermatology department. It is advisable to look for hospitals that specialize in skin conditions or have a good reputation for treating warts and other dermatological issues. You may want to consider hospitals like Hsinchu MacKay Memorial Hospital or National Taiwan University Hospital Hsinchu Branch, where dermatologists can provide comprehensive care.

- When making an appointment, you should specify that you are seeking treatment for condyloma acuminatum or large warts, which will help ensure that you are directed to the appropriate specialist.

4. Referral Process:
- If you encounter difficulties with referrals, it may be beneficial to consult directly with a dermatologist who can assess your condition and recommend the best course of action. In Taiwan, some clinics may allow you to see a specialist without a referral, especially if they have a walk-in service.

In conclusion, while dealing with large warts can be frustrating, there are multiple treatment options available in Taiwan. It is important to consult with a qualified dermatologist who can provide an accurate diagnosis and recommend the most effective treatment plan tailored to your specific needs. Additionally, understanding the costs associated with each treatment option will help you budget accordingly and ensure that you receive the care you need.
