Erectile dysfunction
Hello doctor, I have a mild obesity with a BMI of 27.5 and I have a habit of exercising.
Recently, I have not experienced morning erections.
In the past, when I was intimate with my ex-girlfriend, I could maintain an erection while lying down or sitting, but when I changed positions, it became easier to lose firmness.
I later consulted a doctor who informed me that aside from high bad cholesterol, everything else was normal.
They advised me to change my diet and exercise more.
After about 1 to 2 months of following this advice, I returned to a normal eating pattern.
I then tried to masturbate, averaging about once every two days.
Occasionally, I can maintain an erection while sitting, but when I stand up to change positions, it becomes easier to lose firmness.
Could this still be due to being overweight, or could there be other underlying causes? Thank you.

More Info
Erectile dysfunction (ED) is a common issue that affects many men, and understanding its causes and potential solutions is crucial for maintaining sexual health. Based on your description, it appears that you are experiencing difficulties with maintaining an erection, particularly when changing positions. This can be influenced by several factors, including physical health, psychological aspects, and lifestyle choices.
Causes of Erectile Dysfunction
1. Physical Health: Your BMI of 27.5 categorizes you as overweight, which can contribute to erectile dysfunction. Excess weight can lead to hormonal imbalances, reduced blood flow, and increased risk of cardiovascular diseases, all of which can impair erectile function. Additionally, you mentioned having high bad cholesterol (LDL), which can also affect blood circulation and, consequently, the ability to achieve and maintain an erection.
2. Hormonal Factors: Hormonal imbalances, particularly low testosterone levels, can significantly impact sexual function. While you did not mention any hormonal issues, it's worth considering a check-up to evaluate your testosterone levels, especially if you experience other symptoms like fatigue or decreased libido.
3. Psychological Factors: Stress, anxiety, and depression can all contribute to erectile dysfunction. The psychological aspect of sexual performance is significant; if you are worried about maintaining an erection, this anxiety can create a cycle that makes it even harder to perform.
4. Lifestyle Choices: Your mention of a sedentary lifestyle and dietary habits is also relevant. Regular exercise and a balanced diet can improve overall health, enhance mood, and boost self-esteem, all of which can positively affect sexual performance.
5. Position Changes: The difficulty in maintaining an erection when changing positions could be related to both physical and psychological factors. Physically, certain positions may not provide the same level of stimulation or comfort, leading to a loss of erection. Psychologically, the anxiety of changing positions may also play a role.
Solutions and Recommendations
1. Weight Management: Since your BMI indicates that you are overweight, focusing on weight loss through a combination of diet and exercise can be beneficial. Aim for a balanced diet rich in fruits, vegetables, whole grains, and lean proteins while reducing saturated fats and sugars. Regular physical activity, such as aerobic exercises and strength training, can improve cardiovascular health and enhance erectile function.
2. Regular Check-ups: Regular visits to your healthcare provider to monitor cholesterol levels, blood pressure, and overall health are essential. If you have not already done so, consider discussing your erectile dysfunction with your doctor, who may recommend tests to rule out underlying conditions.
3. Psychological Support: If you suspect that anxiety or stress is contributing to your erectile dysfunction, consider speaking with a mental health professional. Therapy can help address any psychological barriers and improve your sexual confidence.
4. Limit Alcohol and Avoid Smoking: Both alcohol and tobacco can negatively impact erectile function. If you consume alcohol, do so in moderation, and consider quitting smoking if applicable.
5. Experiment with Positions: If certain positions lead to difficulties, try experimenting with different ones that may be more comfortable and stimulating. Communication with your partner about what feels good can also enhance the experience.
6. Medication: If lifestyle changes do not yield improvements, medications such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) may be prescribed by your doctor to help achieve and maintain an erection.
7. Stay Informed: Educate yourself about erectile dysfunction and its causes. Understanding that this is a common issue can help reduce anxiety and improve your confidence.
In conclusion, erectile dysfunction can stem from a variety of factors, including physical health, psychological aspects, and lifestyle choices. By addressing these areas, you can work towards improving your erectile function and overall sexual health. If the problem persists, do not hesitate to seek professional medical advice for further evaluation and treatment options.
