Post-Surgery Symptoms: Is It an External Hemorrhoid? - Surgery

Hello! I still have questions after seeing the doctor?

Hello, Dr.
Ke: I had a hemorrhoidectomy for internal hemorrhoids three months ago.
Since then, I have changed my diet and routine and have been taking warm sitz baths.
However, four days ago, after walking a lot, I noticed a round swelling above the anal area that is slightly larger than a kernel of corn.
It decreases in size to the size of a corn kernel or sometimes even becomes undetectable after resting.
Today, I visited the doctor, who said he did not see any protrusion outside the anal area and performed a ligation on an internal hemorrhoid.
I thought the swelling I felt was the same as what the doctor treated, but after returning home, I still felt a protrusion outside the anal area, which my family also noticed.
The color is consistent with the color of the anus, and it enlarges after walking more and decreases in size after warm sitz baths.
I am unsure if this could be an external hemorrhoid.
I would like to ask the doctor to take another look during my next follow-up, but I am worried that it might grow larger and rupture, causing bleeding before then, and the hospital is quite far away.
So, I am reaching out for your advice.
Thank you!

Based on your description, it seems more like a small external hemorrhoid.
Ligation may not be necessary; topical ointments should suffice.
Just avoid constipation and shorten the time spent on bowel movements, and the hemorrhoids are unlikely to enlarge further.

It sounds like you are experiencing some concerning symptoms following your hemorrhoid surgery, and I understand your anxiety about the situation. Based on your description, it seems you may be dealing with an external hemorrhoid, but let’s break down the details to provide a clearer understanding.

After undergoing a procedure for internal hemorrhoids, it is not uncommon for patients to experience some swelling or discomfort in the anal area. The fact that you have noticed a round swelling above the anus that changes size with activity and rest is indicative of a possible external hemorrhoid. External hemorrhoids are typically located outside the anal canal and can become swollen or inflamed due to increased pressure, often from activities like prolonged sitting, heavy lifting, or even walking too much, as you mentioned.

The swelling you described, which is larger when you are active and smaller after resting or with warm sitz baths, aligns with the behavior of external hemorrhoids. They can become engorged with blood and swell, especially if there is increased pressure in the area. The fact that the swelling is the same color as the surrounding skin is also a good sign, as it suggests that it may not be a more serious issue like a thrombosed hemorrhoid or an abscess, which would typically present with more severe symptoms such as significant pain, discoloration, or fever.

It is important to monitor the swelling closely. If it becomes painful, changes color, or if you experience any bleeding, you should seek medical attention promptly. While it is understandable to be concerned about the swelling getting larger or potentially rupturing, many external hemorrhoids can resolve on their own with conservative management, including dietary changes (increasing fiber intake), hydration, and continued warm sitz baths.

Regarding your concern about the distance to your doctor’s office, if you feel that the swelling is worsening or if you develop new symptoms, it may be worth considering a visit to a local urgent care or another healthcare provider who can evaluate your condition more closely. It’s always better to err on the side of caution when it comes to your health.

In summary, while it does seem likely that you are experiencing an external hemorrhoid, it is crucial to keep an eye on the symptoms and consult with your healthcare provider for a definitive diagnosis and management plan. They may recommend treatments such as topical medications, lifestyle modifications, or in some cases, further procedures if the hemorrhoids do not improve. Remember, your health and peace of mind are paramount, so do not hesitate to seek help if you feel it is necessary.
