Penile retraction

Hello doctor, after I transitioned to farming, I noticed that during the summer or in hot weather, my genitals do not expand outward but rather retract inward.
I'm not sure what could be causing this.
Additionally, I used to never have herpes outbreaks, but now I find that I am prone to them.
Could these issues be related to my work in agriculture? Thank you, doctor.

Ayang, 20~29 year old female. Ask Date: 2020/07/23

Dr. Cai Zhuorong reply Urology

Is this all related to my farming profession? A: There is no medical evidence to suggest that a farming occupation directly causes the symptoms you described.

Genital retraction, particularly in the context of farmers or individuals who work outdoors, can be influenced by several factors, including environmental conditions, physical activity, and even psychological stress. The phenomenon you are experiencing, where your genitalia retracts during hot weather, is not uncommon and can be attributed to a few physiological responses.

Firstly, the human body has a natural mechanism to regulate temperature. When exposed to heat, the body attempts to cool itself down through various means, including vasodilation (the widening of blood vessels) and sweating. However, in some individuals, particularly in response to heat or physical exertion, the cremaster muscle, which surrounds the testicles, may contract. This contraction can cause the testicles to retract closer to the body to maintain a more favorable temperature for sperm production, as the ideal temperature for testicular function is slightly lower than the body's core temperature. This is a protective mechanism to prevent overheating.

Additionally, the physical demands of farming can lead to increased stress and fatigue, which may also contribute to changes in your body's responses. Stress can lead to hormonal changes that might affect various bodily functions, including blood flow and muscle tension, potentially exacerbating the retraction of the genitalia.

Regarding the increased occurrence of herpes outbreaks, it's essential to consider that stress and physical exertion can weaken the immune system. When the body is under stress, whether from physical labor or environmental factors, it may become more susceptible to viral infections, including herpes simplex virus (HSV). If you have a history of herpes, the virus can become reactivated during times of stress, leading to outbreaks. Additionally, exposure to new environments, such as farms, may introduce new pathogens or allergens that could also impact your skin and immune response.

To address your concerns, here are a few recommendations:
1. Stay Hydrated: Ensure you are drinking plenty of fluids, especially in hot weather. Dehydration can exacerbate physical stress and may contribute to discomfort.

2. Wear Appropriate Clothing: Loose-fitting, breathable fabrics can help reduce irritation and allow for better temperature regulation.

3. Manage Stress: Engage in stress-reducing activities, such as exercise, meditation, or hobbies, to help maintain a healthy immune system.

4. Consult a Healthcare Provider: If you continue to experience significant discomfort or if the herpes outbreaks become more frequent or severe, it is advisable to consult a healthcare professional. They can provide guidance on managing herpes and may recommend antiviral medications to reduce the frequency of outbreaks.

5. Monitor for Other Symptoms: Keep an eye on any additional symptoms that may arise, such as pain, swelling, or changes in skin condition, and report these to your healthcare provider.

In summary, the retraction of your genitalia in hot weather and the increased frequency of herpes outbreaks may be linked to physiological responses to heat, stress from physical labor, and potential immune system changes. Taking proactive steps to manage your health and consulting with a healthcare professional can help you address these concerns effectively.
