Foreskin laceration?
Hello Doctor, recently the weather has been hot and humid.
After one shower, I started to experience itching under the foreskin.
Initially, I thought it was just due to inadequate cleaning and didn't pay much attention.
However, after a DIY activity, I noticed a slight stinging sensation under the foreskin the next day, although the predominant feeling was itching.
Later, when I retracted the foreskin, I discovered what seemed to be a small tear.
This itching sensation has persisted for three to four days.
Although I have been keeping the area dry, the itching occurs at irregular intervals.
I have a relatively long foreskin, but I can normally expose the glans during an erection, so I am not considering circumcision.
Should I see a doctor for this, or would applying medication or allowing it to heal naturally be better? I work with chemical agents and sometimes have to work on the ground, which might affect the healing process.
I would appreciate your advice.
Thank you!

Hello: If you want to recover quickly, it's best to seek medical attention, as improper care can lead to worsening conditions and more complications.
Currently, pay attention to wearing looser underwear and monitor for any redness on the skin.
Wishing you good health.

More Info
Prepuce irritation and minor tears can be quite uncomfortable and may arise from various factors, including hygiene practices, skin conditions, or even friction during activities. Given your description of symptoms—itching, slight pain, and the presence of a minor tear—it is essential to address these concerns appropriately to prevent further complications.
Understanding the Symptoms
1. Itching and Irritation: Itching in the prepuce area can be caused by several factors, including irritation from soaps or body washes, moisture accumulation, or even fungal infections. Given that you mentioned a recent DIY activity, it's possible that friction or exposure to irritants could have contributed to your symptoms.
2. Minor Tear: A minor tear in the prepuce can occur due to excessive stretching or friction. If the tear is small and not accompanied by severe pain, bleeding, or signs of infection (such as pus or increased redness), it may heal on its own with proper care.
3. Long Prepuce: Having a longer prepuce can sometimes contribute to irritation, especially if it does not retract easily during hygiene practices. However, since you mentioned that you can expose the glans during an erection, this may not be a significant concern unless it leads to recurrent issues.
Treatment Options
1. Hygiene: Maintaining proper hygiene is crucial. Ensure that the area is cleaned gently with mild soap and water. Avoid harsh soaps or products that may irritate the skin. After washing, pat the area dry rather than rubbing it.
2. Topical Treatments: Over-the-counter hydrocortisone cream can help reduce inflammation and itching. Additionally, applying a barrier cream or ointment (like petroleum jelly) can protect the area and promote healing. However, avoid using any products that contain fragrances or alcohol, as these can exacerbate irritation.
3. Avoid Irritants: Since you work with chemical agents, it’s essential to minimize exposure to these substances, especially if they come into contact with the genital area. Wearing protective clothing and ensuring proper hygiene after work can help reduce irritation.
4. Rest and Recovery: If your work requires positions that may aggravate the area (like kneeling or lying down), try to take breaks or adjust your position to minimize pressure on the prepuce.
When to See a Doctor
While minor irritations can often be managed at home, there are specific situations where you should seek medical advice:
- Persistent Symptoms: If the itching or pain persists beyond a week or worsens, it’s advisable to consult a healthcare professional.
- Signs of Infection: If you notice increased redness, swelling, discharge, or fever, these could be signs of an infection that requires medical treatment.
- Severe Pain: If the pain becomes severe or if the tear does not seem to heal, a medical evaluation is warranted.
Conclusion
In summary, while minor prepuce irritation and tears can often be treated at home with proper hygiene and topical treatments, it is essential to monitor your symptoms closely. Given your work environment and the potential for exposure to irritants, taking preventive measures is crucial. If symptoms persist or worsen, do not hesitate to seek medical attention to ensure proper diagnosis and treatment. Your health and comfort should always be a priority.
