Premenstrual bleeding

Hello doctor, my menstrual period started on July 9, and my average cycle is 30 days.
For the past two months, I have experienced light bleeding and abdominal pain after the fertile window indicated by my app.
However, I found online that ovulation bleeding typically occurs during the days of ovulation.
Additionally, I took a pregnancy test last month and had a gynecological examination, both of which showed no abnormalities.
Is this considered normal on day 20 after my last menstrual period? Thank you.

Lin, 20~29 year old female. Ask Date: 2020/07/29

Dr. Huang Zifei reply Obstetrics and Gynecology

Dear Lin,
Typically, in obstetrics and gynecology, when you present with such complaints, an internal examination and ultrasound will be performed to determine the source of your bleeding.
After that, we will discuss your condition and treatment options.
If you are very concerned, it is advisable to visit an obstetrician-gynecologist.
Alternatively, you may observe one or two more menstrual cycles, and if abnormal bleeding persists, it is recommended to seek consultation with an obstetrician-gynecologist.

More Info

Light bleeding and cramps after ovulation can be concerning, but they are often part of the normal menstrual cycle for many women. Based on your description, it seems you are experiencing what is known as ovulation bleeding, which can occur around the time of ovulation, typically about 14 days before your next period. However, since you mentioned that this bleeding is occurring after your fertile window, it may be worth exploring further.

Ovulation bleeding is usually light spotting that can last from a few hours to a couple of days. It is often accompanied by mild cramping, which can be attributed to the ovulation process itself. When an ovary releases an egg, it can cause a small amount of bleeding due to the rupture of the follicle. This is generally considered normal and is not usually a cause for concern.

However, since you are experiencing this bleeding after your expected ovulation period, it is important to consider other factors. Hormonal fluctuations can lead to unexpected bleeding at various times in your cycle. Stress, changes in weight, or even certain medications can affect your menstrual cycle and lead to irregular bleeding. Additionally, conditions such as polycystic ovary syndrome (PCOS), fibroids, or endometriosis can also cause abnormal bleeding and cramps.

Given that you have already undergone a pregnancy test and a gynecological examination without any abnormalities being found, it is likely that what you are experiencing is not indicative of a serious issue. However, if the bleeding persists, becomes heavier, or is accompanied by severe pain, it would be advisable to consult with your healthcare provider for further evaluation. They may recommend additional tests, such as an ultrasound or blood tests, to rule out any underlying conditions.

In summary, light bleeding and cramps after ovulation can be normal for some women, especially if they are mild and short-lived. However, since your symptoms are occurring outside of the typical ovulation window, it is important to monitor them closely. If you have any concerns or if the symptoms change in intensity or frequency, seeking medical advice is always a prudent step. Your health and peace of mind are paramount, and a healthcare professional can provide personalized guidance based on your specific situation.
