Presence of blood in semen?
Hello, Dr.
Du.
I would like to ask about my usual DIY habits.
My semen color is typically normal, either milky white or light yellow.
However, today during DIY, I noticed that while the semen was still the normal color, there were some very fine red streaks in the middle of the semen, with a very small amount, similar to a dot from a water-based pen on white paper.
I used a tissue to gently wipe it and confirmed it was fresh blood.
Why is this happening? Is it due to a rupture of microvessels or what is commonly referred to as hematospermia? I didn't feel any pain during ejaculation before, but this time I experienced repeated firmness and softness twice before ejaculation, and it felt like there was some pressure.
Could this be the cause? Is there a problem with my urinary tract? This has never happened before, and I have not been promiscuous! Could you please clarify this for me?
Ayou, 30~39 year old female. Ask Date: 2020/08/03

Hello: The presence of blood in semen is referred to as hematospermia, and it is generally advisable to seek medical attention.
However, if the condition is not severe and there are no other symptoms, you may observe a few more ejaculations; if there is no blood, it indicates that the bleeding has stopped.
For potential causes, you can refer to http://www.shutien.org.tw/library_detail.aspx?healthid=324.
Severe diseases are often accompanied by other symptoms, and hematospermia can recur, sometimes after a gap of six months to a year.
Please take care of yourself.
Wishing you good health.

When it comes to the presence of blood in semen, a condition known as hematospermia, it can understandably cause concern for many men. The appearance of red streaks or spots in the semen can be alarming, but it is important to understand that there are various potential causes, and not all of them are serious.
Firstly, the presence of blood in semen can result from several benign conditions. One common cause is the rupture of small blood vessels, which can occur due to physical activities, including sexual activity or masturbation. If you experienced some form of pressure or trauma during ejaculation, it is possible that this could have led to minor bleeding from the prostate or seminal vesicles, which are involved in semen production.
In your case, you mentioned that the blood was minimal and that you did not experience any pain during ejaculation, which is a positive sign. Hematospermia can also be associated with inflammation or infection in the reproductive tract, such as prostatitis (inflammation of the prostate) or epididymitis (inflammation of the epididymis). These conditions can sometimes cause discomfort or pain, but not always.
Another potential cause could be a benign enlargement of the prostate, known as benign prostatic hyperplasia (BPH), which is common in older men. However, since you did not mention your age, it is difficult to assess the likelihood of this condition being a factor.
While the presence of blood in semen can be alarming, it is essential to consider the context. If this is an isolated incident and you do not experience any other symptoms such as pain, swelling, or changes in urination, it may not be a cause for immediate concern. However, if the blood persists, increases in quantity, or is accompanied by other symptoms, it would be prudent to seek medical evaluation.
In terms of diagnostic steps, a healthcare provider may recommend a physical examination, a urinalysis, and possibly imaging studies or further tests to rule out any underlying conditions. It is also important to discuss your sexual history and any other symptoms you may have experienced.
In conclusion, while the presence of blood in semen can be concerning, it is often due to benign causes such as minor trauma or inflammation. However, if this symptom persists or is accompanied by other concerning signs, it is essential to consult a healthcare professional for a thorough evaluation. Regular check-ups and open communication with your healthcare provider about any changes in your health are key to maintaining your well-being.
