Epididymitis
Hello, doctor.
Following up on the issue discussed in #159312, I am currently seeing a urologist at Taipei Medical University Hospital.
I still have pain in my right testicle, left lower abdomen, and left thigh.
Is it normal to experience these symptoms while I have not yet finished my medication (two weeks of Adalimumab capsules, Xinjingnuo, and seven days of Depyretin)? Additionally, can I exercise during the treatment period? Is it safe to walk long distances or hike?
Howard, 30~39 year old female. Ask Date: 2020/08/07
Dr. Du Shixi reply Urology
Hello: Theoretically, discomfort should be felt in the lower abdomen on the same side, but the thigh should not be affected.
Is your epididymitis on the left or right side? If it is on the left, you may experience discomfort in the left lower abdomen.
After completing the medication, you should return to normal.
Currently, moderate exercise is acceptable, but prolonged inactivity is not advisable.
Wishing you good health.

More Info
Epididymitis is an inflammation of the epididymis, a tube located at the back of the testicle that stores and carries sperm. This condition can be caused by various factors, including bacterial infections, sexually transmitted infections, or even non-infectious causes such as trauma or certain medications. Understanding the symptoms, treatment options, and exercise guidelines is crucial for effective management and recovery.
Symptoms of Epididymitis
The symptoms of epididymitis can vary but typically include:
- Pain and Swelling: The most common symptom is pain in the affected testicle, which may radiate to the lower abdomen or groin. Swelling of the epididymis may also occur.
- Discomfort: Patients often report a feeling of heaviness in the affected testicle.
- Urinary Symptoms: These may include pain during urination, increased frequency of urination, or a discharge from the penis.
- Fever and Chills: In some cases, systemic symptoms like fever may be present, indicating a more severe infection.
Treatment for Epididymitis
The treatment for epididymitis largely depends on the underlying cause:
- Antibiotics: If the condition is caused by a bacterial infection, antibiotics are the primary treatment. Commonly prescribed antibiotics include doxycycline or ciprofloxacin, depending on the suspected pathogen.
- Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort.
- Rest and Support: Patients are often advised to rest and wear supportive underwear to reduce discomfort.
- Ice Therapy: Applying ice packs to the affected area can help reduce swelling and pain.
Exercise Guidelines During Treatment
During the treatment of epididymitis, it is essential to listen to your body and avoid activities that may exacerbate your symptoms. Here are some general guidelines regarding exercise:
1. Avoid Strenuous Activities: It is advisable to refrain from heavy lifting, intense workouts, or activities that put strain on the lower abdomen and groin, such as running or jumping, until your symptoms improve.
2. Gentle Movements: Light activities such as walking may be acceptable, but you should monitor your pain levels. If walking causes increased discomfort, it may be best to limit this activity.
3. Listen to Your Body: If you experience pain during any activity, it is crucial to stop and rest. Pushing through pain can lead to further complications or prolong recovery.
4. Consult Your Doctor: Since you are currently under treatment with medications like Adalimumab and Depyretin, it is essential to consult your healthcare provider before resuming any exercise regimen. They can provide personalized advice based on your specific condition and treatment progress.
Conclusion
In summary, while experiencing symptoms of epididymitis, it is normal to have discomfort in the affected areas, especially during the initial stages of treatment. However, it is crucial to follow your doctor's advice regarding medication and activity levels. Engaging in light, non-strenuous activities may be beneficial, but always prioritize your comfort and recovery. If you have any concerns about your symptoms or the appropriateness of specific exercises, do not hesitate to reach out to your healthcare provider for guidance.
