Prostatitis

Hello Dr.
Lee, I would like to ask how long it usually takes to recover from prostatitis.
I believe I have prostatitis; after ejaculation, I experience a strong urge to urinate, and my urethra feels sensitive and uncomfortable.
Sometimes, I feel pressure in the area of the prostate, but not every ejaculation results in symptoms.
Sometimes, I recover the same day, while other times, symptoms persist for over a week, and these symptoms only appear after ejaculation.
I am currently abroad and find it inconvenient to see a doctor.
A friend who practices traditional Chinese medicine listened to my symptoms and believes it is prostatitis, and he prescribed me herbal medicine, which I have been taking for about three months.
However, I still feel some sensitivity and discomfort in the urethra after ejaculation, which is quite frustrating.
The symptoms are milder than before, but I feel that my recovery is very slow.
I exercise daily, drink plenty of water, limit my sitting time to no more than eight hours, have almost completely stopped drinking coffee, and rarely smoke.
I still drink tea, and I have forced myself to ejaculate only once every 10-14 days to minimize the frequency.
I would like to ask Dr.
Lee the following questions: 1.
Are my symptoms indicative of prostatitis? 2.
I read online that recovery can take about two months, but my recovery seems slow; could it be because herbal treatment takes longer? 3.
If I do not heal, should I switch to Western medicine, or should I continue with herbal treatment and allow my body to recover slowly? 4.
After recovery, how can I determine the appropriate frequency of ejaculation to prevent recurrence? Thank you.

Eric, 30~39 year old female. Ask Date: 2020/08/07

Dr. Li Xinyi reply Urology

Based on your description, it seems related to ejaculation.
I recommend that the duration of sexual activity should not be too long.
There are no specific recommendations regarding frequency, as it varies from person to person and depends on your current physical condition.
It may be necessary to consult a urologist for an examination and further discussion with the physician, as I cannot provide a diagnosis or potential treatment options without an examination.
Your symptoms are not specific enough for a definitive diagnosis.
Possible causes include untreated urethritis, dietary zinc deficiency, poor pelvic circulation, prostatitis, chronic pelvic pain syndrome, among others.

Prostate inflammation, commonly referred to as prostatitis, can be a challenging condition to manage, and your concerns are valid. Let’s address your questions systematically while providing a comprehensive overview of the condition, its treatment options, and recovery expectations.

1. Is this symptom indicative of prostatitis?
Based on your description, the symptoms you are experiencing—such as sensitivity in the urethra after ejaculation, pressure in the pelvic area, and variations in symptom intensity—are consistent with prostatitis. This condition can manifest in various forms, including acute bacterial prostatitis, chronic bacterial prostatitis, and chronic pelvic pain syndrome (CPPS), which is the most common form. The intermittent nature of your symptoms, particularly their association with ejaculation, suggests that you may be dealing with CPPS, which is often more challenging to diagnose and treat.

2. Recovery time and treatment efficacy:
The recovery time for prostatitis can vary significantly depending on the underlying cause and the treatment approach. While some sources suggest that symptoms can improve within two months, this is not a universal timeline. The effectiveness of treatment can also differ between Western medicine and traditional Chinese medicine (TCM). TCM may take longer to show results, as it often focuses on holistic healing and balancing the body's systems rather than targeting specific symptoms. If you have been on herbal treatment for three months and are still experiencing discomfort, it may be worth considering a consultation with a urologist for a more targeted approach, possibly including antibiotics or anti-inflammatory medications.

3. Should I switch to Western medicine if symptoms persist?
If your symptoms do not improve with TCM, it may be beneficial to explore Western medical treatments. Antibiotics are typically prescribed for bacterial prostatitis, while other medications, such as alpha-blockers or anti-inflammatory drugs, may be used for CPPS. It is essential to have a thorough evaluation by a healthcare provider who can assess your condition and recommend appropriate treatment options based on your specific symptoms and medical history.
4. Post-recovery management and ejaculation frequency:
Once you achieve symptom relief, determining the appropriate frequency of ejaculation can be crucial in preventing recurrence. While there is no one-size-fits-all answer, many healthcare providers suggest that regular ejaculation (whether through sexual activity or masturbation) can help maintain prostate health by promoting drainage of prostatic fluid. However, the frequency should be tailored to your comfort level and symptomatology. It may be helpful to keep a symptom diary to track how your body responds to different frequencies of ejaculation, allowing you to find a balance that minimizes discomfort while promoting prostate health.

In addition to medication, lifestyle modifications can play a significant role in managing prostatitis. Staying hydrated, avoiding irritants such as caffeine and alcohol, and maintaining a healthy weight can contribute to symptom relief. Regular exercise, as you mentioned, is also beneficial, as it can improve circulation and reduce pelvic tension.

In summary, prostatitis can be a complex condition with varying recovery times and treatment responses. If your symptoms persist despite TCM, consider seeking a urologist's opinion for a comprehensive evaluation and potential adjustment of your treatment plan. Monitoring your symptoms and adjusting your lifestyle can also aid in recovery and help prevent future flare-ups.
