Foreign body on the tongue

I have recently been feeling a sensation of something foreign in my throat, as if something has grown there.
After taking some pictures, I noticed a protrusion at the base of my tongue: https://i.imgur.com/20ErGC2.jpg https://i.imgur.com/LijnhPd.jpg.
Is this normal? I have a history of gastroesophageal reflux disease (GERD), but this doesn't seem to look like something caused by GERD.
Could you please help me confirm this?

asd, 20~29 year old female. Ask Date: 2020/08/09

Dr. Zheng Jueyi reply Otolaryngology

Hello: Upon initial examination, it appears to be circumvallate papillae at the base of the tongue.
If the pain persists or if there are any discomforting symptoms, it is recommended to seek medical evaluation for confirmation.
Wishing you peace and health! Dr.

More Info

When experiencing a sensation of a foreign body in the throat, it can be concerning and may lead to anxiety about potential underlying health issues. The sensation you describe, often referred to as "globus sensation," can arise from various causes, and it is essential to consider both physical and psychological factors.

Possible Causes of Throat Sensation
1. Gastroesophageal Reflux Disease (GERD): Given your history of GERD, this condition could contribute to the sensation of something being stuck in your throat. Acid reflux can irritate the lining of the esophagus and throat, leading to inflammation and a feeling of tightness or a lump.

2. Postnasal Drip: Allergies or sinus infections can lead to mucus dripping down the back of the throat, causing irritation and a sensation of fullness or obstruction.

3. Muscle Tension: Stress and anxiety can lead to muscle tension in the throat, which may manifest as a feeling of tightness or a lump. This is often exacerbated by habits such as throat clearing or swallowing.

4. Throat Conditions: Conditions such as pharyngitis, tonsillitis, or even benign growths like cysts or polyps can create a sensation of obstruction.
5. Structural Abnormalities: In some cases, anatomical variations or growths in the throat or tongue base can lead to unusual sensations.

Evaluation of the Images
The images you provided show a protrusion at the base of the tongue. While I cannot diagnose based solely on images, some considerations include:
- Normal Anatomical Variants: Sometimes, what appears as a protrusion can be a normal anatomical structure, such as a lingual tonsil or a normal variant of the tongue's anatomy.

- Benign Growths: If the protrusion is new and has changed in size or shape, it may warrant further evaluation to rule out benign growths or cysts.

- Inflammation or Infection: If there is accompanying redness, swelling, or pain, this could indicate an inflammatory process that may need treatment.

Recommendations
1. Consult an ENT Specialist: Given your symptoms and the presence of a protrusion, it is advisable to consult an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, possibly including a laryngoscopy, to visualize the throat and determine the nature of the protrusion.

2. Manage GERD: If GERD is contributing to your symptoms, optimizing your treatment plan may alleviate some discomfort. This could involve dietary changes, medications, or lifestyle modifications.

3. Monitor Symptoms: Keep track of any changes in your symptoms, including the size of the protrusion, any pain, difficulty swallowing, or changes in voice. This information will be helpful for your healthcare provider.

4. Consider Psychological Factors: If stress or anxiety is a contributing factor, consider discussing this with a healthcare provider. Techniques such as mindfulness, relaxation exercises, or therapy may help reduce muscle tension and alleviate symptoms.

Conclusion
While the sensation of a foreign body in the throat can be distressing, many causes are benign and manageable. It is crucial to seek professional evaluation to rule out any serious conditions and to receive appropriate treatment for any underlying issues. Your health and peace of mind are paramount, so do not hesitate to reach out to a healthcare provider for further assistance.
