Can I get pregnant?

Hello doctor, I touched my girlfriend today, and I ejaculated 2 hours ago.
I washed and wiped my penis, but there was a little leakage on my underwear.
Two hours later, during more touching, a small amount of pre-ejaculate fluid came out.
My question is, does this contain sperm from the ejaculation that occurred 2 hours earlier? If I inserted my fingers into her vagina and touched the semen that was on my underwear from 2 hours ago, could this lead to pregnancy?

Zu, 20~29 year old female. Ask Date: 2020/08/09

The question of whether pregnancy can occur from pre-ejaculate fluid (pre-cum) and skin contact is a common concern among sexually active individuals. To address this question, it is essential to understand the biology of sperm and the mechanisms of conception.

Pre-ejaculate fluid is produced by the Cowper's glands and serves several purposes, including lubricating the urethra and neutralizing acidity in the urethra to protect sperm. While pre-ejaculate itself typically does not contain sperm, there is a possibility that it can pick up sperm left in the urethra from a previous ejaculation. This is particularly relevant if ejaculation occurred shortly before the pre-ejaculate is released.

In your scenario, if you had ejaculated two hours prior and then engaged in sexual activity that involved skin contact and the potential transfer of pre-ejaculate, there are a few factors to consider:
1. Sperm Viability: Sperm can live outside the body for a limited time, depending on the environment. Sperm on skin or clothing can dry out quickly, which significantly reduces their viability. However, if the sperm were still moist, there is a slight chance they could remain viable for a short period.

2. Transfer of Sperm: If there was any sperm present on your skin or underwear, and you then touched your partner's vagina, there is a theoretical risk of transferring viable sperm. However, the likelihood of this resulting in pregnancy is low, especially if there was a significant time gap and the sperm had dried.

3. Conception Mechanics: For pregnancy to occur, sperm must travel through the cervix into the uterus and fertilize an egg. This process requires a certain number of viable sperm to be present in a suitable environment. The chances of sperm successfully traveling this route from skin contact are minimal, but not impossible.

4. Timing and Ovulation: The likelihood of pregnancy also depends on your partner's menstrual cycle. If she is not ovulating, the chances of conception are significantly reduced. Ovulation typically occurs around the midpoint of the menstrual cycle, and sperm can survive in the female reproductive tract for up to five days, but the window for fertilization is limited to a few days each cycle.

In summary, while it is theoretically possible to become pregnant from pre-ejaculate and skin contact, the actual risk is quite low, especially given the circumstances you described. If you or your partner are concerned about the possibility of pregnancy, consider using emergency contraception if within the appropriate time frame and consult with a healthcare provider for personalized advice.

For future sexual encounters, it is advisable to use reliable contraception methods to minimize the risk of unintended pregnancy. Barrier methods, such as condoms, not only help prevent pregnancy but also reduce the risk of sexually transmitted infections (STIs). If you have further questions or concerns about sexual health or contraception, consulting a healthcare professional can provide you with accurate information and guidance tailored to your situation.
