Intermenstrual bleeding
Hello Doctor, I usually do not keep track of my menstrual cycle.
Four days ago, I suddenly noticed brownish discharge with a bit of blood (without any odor).
For the second and third days, the bleeding became more noticeable, and today, on the fourth day, the bleeding has decreased.
However, it has already exceeded the typical 1-3 days of ovulation bleeding.
I also feel a slight dull pain in my lower abdomen.
My recent sleep schedule has been irregular, going to bed after 3 AM, but I have been getting a full seven hours of sleep.
After researching, I found that most other symptoms are related to sexual activity.
Since I have never engaged in sexual activity, should I be less concerned? Please help clarify this for me.
Thank you!
Niming, 20~29 year old female. Ask Date: 2020/08/10

Abnormal bleeding can be a source of significant concern for many women, and understanding its causes is crucial for addressing any underlying issues. Your description of experiencing brownish discharge with blood, followed by a more pronounced bleeding that has lasted longer than the typical duration of ovulation bleeding, raises several points for consideration.
Firstly, it’s important to note that abnormal bleeding can stem from various factors, including hormonal imbalances, stress, lifestyle changes, and even underlying medical conditions. In your case, the irregular sleep patterns and stress you mentioned could be contributing factors. Disruptions in sleep can affect the body’s hormonal balance, potentially leading to irregular menstrual cycles and abnormal bleeding. The body’s hormonal regulation is delicate, and factors such as stress and sleep deprivation can lead to fluctuations in estrogen and progesterone levels, which are critical for maintaining a regular menstrual cycle.
The fact that you have not engaged in sexual intercourse does reduce the likelihood of certain causes of abnormal bleeding, such as pregnancy-related issues or sexually transmitted infections. However, it does not completely rule out other potential causes. For instance, conditions such as polycystic ovary syndrome (PCOS), uterine fibroids, or endometriosis can lead to abnormal bleeding regardless of sexual activity. Additionally, ovulation bleeding, which is often light and occurs around the middle of the menstrual cycle, can sometimes be mistaken for abnormal bleeding, especially if it lasts longer than expected.
The presence of a dull ache in your lower abdomen could also be indicative of ovulation pain (mittelschmerz), which some women experience during ovulation. However, if the pain persists or worsens, it is advisable to consult a healthcare provider for further evaluation.
Given that your bleeding has lasted longer than the typical duration for ovulation bleeding and is accompanied by discomfort, it would be prudent to seek medical advice. A healthcare provider may recommend a pelvic examination, ultrasound, or blood tests to assess hormone levels and rule out any underlying conditions. These evaluations can help clarify the cause of your symptoms and guide appropriate treatment options.
In summary, while your lack of sexual activity may alleviate some concerns, it is essential to consider other potential causes of abnormal bleeding. Lifestyle factors such as stress and sleep patterns can significantly impact menstrual health, and addressing these may help restore regularity. However, persistent or unusual symptoms warrant professional evaluation to ensure that any serious conditions are identified and managed appropriately. It is always better to err on the side of caution when it comes to your health, so do not hesitate to reach out to a healthcare provider for personalized advice and support.
