Could this situation be due to a fungal infection in the genital area?

Hello, Doctor! I have not engaged in sexual intercourse.
Recently, I have been experiencing intense itching in my genital and anal areas, which has been so severe that it keeps me awake at night.
On August 7, 2020 (last week), I visited a gynecologist, and she prescribed me one dose of fluconazole 150 mg.
I took it immediately upon returning home.
On August 11, 2020, which is the fifth day of treatment, I still felt some itching, so I went back to the gynecologist to ask if I could take another 150 mg of fluconazole since the itching had not completely resolved, but it was much better than on the first day.
Therefore, she prescribed another 150 mg of fluconazole.
My questions are: What could be the cause of the itching in my case? If it is a fungal infection, how much fluconazole would I need to take to completely eradicate the fungus? Another question I have is whether I can take the second dose of fluconazole 150 mg prescribed by the doctor today, or should I wait for 7 days before taking it? Thank you.

Dr. Lin Manying reply Obstetrics and Gynecology

Ms.
Ines: Hello, based on the prescribed medication, it appears to be a fungal infection.
If there are concerns regarding liver function, it is recommended to take one capsule of fluconazole again after 5-7 days.

Persistent itching in the genital and anal areas can indeed be a sign of a fungal infection, particularly a yeast infection caused by Candida species. This type of infection is common and can occur in both men and women, often presenting with symptoms such as intense itching, irritation, and sometimes a discharge. In your case, the fact that you have not engaged in sexual activity does not rule out the possibility of a yeast infection, as these infections can occur due to various factors, including antibiotic use, hormonal changes, or even wearing tight or non-breathable clothing.

Fluconazole is an antifungal medication commonly prescribed to treat yeast infections. The standard dosage for a single episode of vaginal candidiasis is typically 150 mg, which you have already taken. If you are still experiencing itching after five days, it is possible that the initial dose was not sufficient to completely eradicate the infection, or there may be another underlying cause for your symptoms.

Regarding your question about taking another 150 mg dose of fluconazole, it is generally recommended to wait at least seven days after the initial dose before taking another one unless directed otherwise by your healthcare provider. This is to prevent potential side effects and to allow the medication to work effectively. Since you mentioned that the itching has improved but is still present, it would be wise to consult your healthcare provider again before taking another dose. They may want to evaluate your symptoms further, possibly perform a physical examination, or even conduct tests to confirm the presence of a fungal infection or rule out other conditions such as bacterial infections or skin irritations.

In addition to medication, maintaining proper hygiene and avoiding irritants can help alleviate symptoms. Wearing loose-fitting, breathable cotton underwear and avoiding scented products in the genital area can also be beneficial. If the itching persists despite treatment, further investigation may be necessary to identify any other underlying issues, such as allergies, dermatitis, or other infections.

In summary, while fluconazole is effective for treating yeast infections, it is essential to follow your healthcare provider's guidance regarding dosage and timing. If symptoms continue or worsen, further evaluation is warranted to ensure appropriate treatment and management of your condition.
