Persistent Pain from Glans Friction: Seeking Solutions - Urology

Friction on the glans can be painful?

Hello doctor, a few days ago I experienced pain due to friction in the area of the glans and frenulum, so I visited a family medicine doctor.
However, the doctor couldn't determine the cause; there were no signs of trauma, redness, swelling, or any unusual discharge.
The doctor prescribed a three-day course of oral anti-inflammatory medication and a topical ointment.
I have completed the medication and have been applying the ointment twice daily for four days, but I still experience discomfort when there is friction.
It's not very painful, just somewhat uncomfortable.
What should I do?

Hello: There may be a small wound, so it is important to keep it clean and protected for a few days.
Currently, sexual activity should be avoided to prevent worsening of symptoms.
Wishing you good health.

More Info

Persistent pain in the glans area, particularly after friction, can be quite distressing and may indicate several underlying issues. Given your description, it seems that you have already consulted a family physician who prescribed anti-inflammatory medication and topical ointment, but unfortunately, the pain persists. Here are some considerations and potential next steps you might take.

Possible Causes of Pain
1. Friction or Irritation: The most straightforward explanation for your discomfort could be friction or irritation from clothing, sexual activity, or even excessive cleaning. This can lead to inflammation of the sensitive skin around the glans.

2. Balanitis: This is an inflammation of the glans (the head of the penis) that can be caused by infections (bacterial or fungal), irritants, or poor hygiene. While you mentioned no visible signs of redness or discharge, mild cases can sometimes present without these symptoms.

3. Allergic Reaction: Sometimes, personal care products, soaps, or even condoms can cause allergic reactions that lead to discomfort in the genital area.

4. Dermatological Conditions: Conditions such as psoriasis or eczema can also affect the genital area, leading to discomfort or pain.

5. Nerve Sensitivity: If there has been any trauma or irritation to the area, it could lead to increased nerve sensitivity, resulting in persistent discomfort even after the initial cause has been resolved.

Recommended Actions
1. Follow-Up with a Specialist: Since the pain persists despite treatment, it may be beneficial to see a urologist or a dermatologist who specializes in genital conditions. They can perform a more thorough examination and may suggest additional tests, such as swabs or cultures, to rule out infections.

2. Avoid Irritants: In the meantime, try to avoid any potential irritants. This includes harsh soaps, lotions, or any products that may come into contact with the area. Opt for mild, unscented products.

3. Gentle Hygiene Practices: Maintain good hygiene but avoid excessive scrubbing or washing that could further irritate the area. Use lukewarm water and a gentle cleanser.

4. Consider Protective Measures: If friction is a concern, consider wearing looser-fitting underwear or protective garments that can reduce friction during daily activities.

5. Monitor Symptoms: Keep track of any changes in your symptoms, including the intensity of the pain, any new symptoms, or changes in the appearance of the skin. This information can be valuable for your healthcare provider.

6. Pain Management: If the discomfort is significant, over-the-counter pain relief medications such as ibuprofen or acetaminophen may help manage the pain temporarily. However, consult with your doctor before starting any new medication.

7. Avoid Sexual Activity: If the pain is exacerbated by sexual activity, it may be wise to abstain until you have a clearer understanding of the cause and have consulted with a specialist.

Conclusion
Persistent pain in the glans area can be caused by various factors, and while initial treatments may not have provided relief, further evaluation by a specialist is crucial. They can help identify the underlying cause and recommend appropriate treatment options. In the meantime, practicing gentle hygiene, avoiding irritants, and monitoring your symptoms can help manage discomfort. Remember, it’s important to advocate for your health and seek further medical advice if your symptoms do not improve.
