Abnormal Bleeding After Taking Emergency Contraceptive Pills - Obstetrics and Gynecology

Bleeding occurred 8 days after taking "Yueji" (a type of medication or supplement)?

Hello, doctor.
I would like to ask you about my situation.
After taking the emergency contraceptive pill on July 23, I experienced bleeding from July 29 to August 5.
I went to the pharmacy to buy the pre-conception pill (Yueji), and the pharmacist said that the bleeding from July 29 to August 5 counts as a withdrawal bleed, so I could start taking the pre-conception pill on August 6.
I took it from August 6 to August 13 without any bleeding or discomfort.
However, on August 14, I started to bleed again until August 20, which was a small amount of brown blood.
On August 21, the blood turned bright red.
Before August 21, all the bleeding was without discomfort, but the bright red blood on August 21 felt similar to my menstrual period, with a sense of heaviness in my abdomen and lower back pain.
According to the pharmacist, the contraceptive effect would begin on August 7 and would be effective by August 20, so I considered August 18 as the start of my safe period, and I had unprotected intercourse on August 21.
I would like to ask the following questions:
1.
Is the bleeding from August 14 to August 20 due to the pre-conception pill? Is this brown discharge normal?
2.
Why did the bleeding suddenly change to bright red on August 21? According to the pharmacist, my period should not arrive until August 27, right?
3.
What is the likelihood of pregnancy from unprotected intercourse on August 21?
4.
How can I differentiate between brown discharge or spotting due to ectopic pregnancy versus side effects from taking the contraceptive pill?
5.
Is the bright red bleeding on August 21 my period? Should I see a gynecologist?

Ning, 20~29 year old female. Ask Date: 2020/08/21

Hello, the user: If you experience menstruation or bleeding during the usage period, it is recommended to continue taking the medication as scheduled.
Once your body adapts to the contraceptive, abnormal bleeding should cease.
However, if irregular bleeding persists for more than three months or if there is heavy bleeding, you should consult a physician for advice.
Thank you! Wishing you peace and health.

More Info

Understanding abnormal bleeding after taking emergency contraceptive pills can be quite complex, as various factors can influence menstrual cycles and bleeding patterns. Let's address your questions one by one.

1. Is the bleeding from 8/14 to 8/20 due to the emergency contraceptive pill? Is the brown discharge normal?
Yes, the bleeding you experienced from 8/14 to 8/20 could be a side effect of taking the emergency contraceptive pill. Emergency contraceptives can cause changes in your menstrual cycle, leading to what is often referred to as "withdrawal bleeding." Brown discharge is typically old blood that has taken longer to exit the body and is generally considered normal. It can occur as a result of hormonal fluctuations caused by the pill.

2. Why did the bleeding suddenly change to bright red on 8/21? Shouldn't my period be due on 8/27?
The change to bright red blood on 8/21 could indicate the start of your menstrual period. The timing of your cycle can be affected by the emergency contraceptive pill, which may cause your period to come earlier or later than expected. While the pill is designed to prevent pregnancy, it can also disrupt your normal hormonal balance, leading to unexpected bleeding patterns. If your cycle is typically regular, the bright red blood could signify the onset of your period, even if it is earlier than anticipated.

3. What is the likelihood of pregnancy from unprotected intercourse on 8/21?
If you had unprotected intercourse on 8/21, the likelihood of pregnancy depends on several factors, including your ovulation cycle. If you were in your fertile window, there is a risk of pregnancy. However, since you were taking the emergency contraceptive pill, it is designed to prevent ovulation and reduce the chances of pregnancy. If you are concerned about the risk of pregnancy, consider taking a home pregnancy test if your period does not arrive as expected.

4. How can I differentiate between brown discharge or spotting and potential ectopic pregnancy symptoms?
Brown discharge or spotting is often a normal occurrence, especially after hormonal changes from contraceptive use. However, if you experience severe abdominal pain, shoulder pain, or heavy bleeding, these could be signs of an ectopic pregnancy, which is a medical emergency. Other symptoms may include dizziness or fainting. If you have any concerns about your symptoms or if they worsen, it is essential to seek medical attention promptly.

5. Is the bright red bleeding on 8/21 my period? Should I see a gynecologist?
The bright red bleeding on 8/21 could very well be your period, especially if it is accompanied by typical menstrual symptoms like cramping. However, if you are uncertain or if the bleeding is unusually heavy or prolonged, it is advisable to consult a gynecologist. They can provide a thorough evaluation and ensure that everything is normal.

In summary, hormonal contraceptives can significantly affect your menstrual cycle, leading to variations in bleeding patterns. If you have any concerns about your health or experience any unusual symptoms, it is always best to consult with a healthcare professional for personalized advice and care.
