I would like to inquire about severe hair loss issues, specifically regarding hair loss as a side effect resulting from other surgeries and their associated complications?

I would like to inquire about the issue of severe hair loss, which has been a side effect resulting from other surgeries.
Hello, doctor.
I am currently 27 years old and will soon turn 28.
I work as a regular office worker and part-time media journalist.
Due to weight issues, I underwent my first bariatric surgery (sleeve gastrectomy) in 2014-2015, and then I had a second bariatric surgery (gastric bypass) in 2019.
I began to notice hair loss around 2015-2016, and I started taking multivitamins immediately after the surgeries, which I continue to do.
However, after my second surgery last year, I noticed that the hair loss has become even more severe! After washing my hair every day, I have to clean the bathroom sink, where I find about 50 or more strands of hair, which I can only describe as a handful.
I have to clean my room approximately every two days due to the alarming amount of hair loss.
Because I have long hair, I need to comb it frequently, and I notice that even with light combing, there are always 2 to 10 strands of hair on the comb.
Comparing with photos from a few years ago, I can see a significant reduction in overall hair volume.
My hair now looks very flat and thin.
I am unsure whether I should consider taking some of the currently FDA-approved oral medications.
I found that there are mainly four medications: Rogaine (Minoxidil), Finasteride 1mg, Proscar (Finasteride 5mg), and Dutasteride 0.5mg.
The first two are mentioned more frequently in online articles, while the latter two seem to be newer medications.
I would like to hear your thoughts on my situation and if you could provide me with some treatment plans or references for the future.
Thank you.

Hello: For a detailed introduction to Finasteride and Dutasteride, you can refer to the article by Med's good friend at https://www.medpartner.club/dutasteride-finasteride-comparison-androgenetic-alopecia/.
The indications for the four medications mentioned are all for androgenetic alopecia.
It is recommended that you consult a dermatologist nearby to diagnose the cause of hair loss and discuss treatment options at http://www.derma.org.tw/user_finddoctor.php.
Thank you!

Understanding hair loss after bariatric surgery is crucial, especially for individuals who have undergone significant weight loss procedures like sleeve gastrectomy or gastric bypass. The experience of hair loss can be distressing, and it is often multifactorial, involving nutritional deficiencies, hormonal changes, and the physical stress of surgery itself.

Causes of Hair Loss After Bariatric Surgery:
1. Nutritional Deficiencies: One of the most common reasons for hair loss post-surgery is nutritional deficiency. Bariatric surgery alters the digestive system, which can lead to malabsorption of essential nutrients. Key nutrients that are vital for hair health include protein, iron, zinc, and vitamins such as B12 and D. A lack of these nutrients can lead to a condition known as telogen effluvium, where hair follicles prematurely enter the resting phase, leading to increased shedding.

2. Physical Stress: The body undergoes significant stress during and after surgery, which can trigger hair loss. This is a natural response, as the body reallocates resources to heal and recover from the surgical procedure. Stress-induced hair loss is typically temporary, but it can be exacerbated by other factors such as nutritional deficiencies.

3. Hormonal Changes: Weight loss can lead to changes in hormone levels, including those related to androgens, which can affect hair growth. In some cases, individuals may experience an increase in androgen levels, leading to conditions like androgenetic alopecia, particularly in those predisposed to it.

4. Genetic Predisposition: If there is a family history of hair loss, individuals may be more susceptible to experiencing hair thinning or loss after surgery, regardless of the nutritional or hormonal changes.

Treatment Options:
1. Nutritional Support: The first step in addressing hair loss after bariatric surgery is to ensure adequate nutrition. Patients should work closely with a dietitian to develop a meal plan that includes sufficient protein and micronutrients. Supplements may be necessary to address deficiencies, particularly in iron, vitamin D, and B vitamins.

2. Medications: For those experiencing significant hair loss, medications like Finasteride and Dutasteride can be considered. These medications are primarily used to treat androgenetic alopecia by inhibiting the action of androgens on hair follicles. However, it is essential to consult with a healthcare provider before starting any medication to ensure it is appropriate for your specific situation.

3. Topical Treatments: Minoxidil is a topical treatment that can help stimulate hair growth in some individuals. It is available over the counter and can be used in conjunction with other treatments.

4. Hair Care Practices: Gentle hair care practices can help minimize further hair loss. This includes using a wide-toothed comb, avoiding tight hairstyles, and reducing the use of heat styling tools.

5. Monitoring and Follow-Up: Regular follow-up with healthcare providers is crucial to monitor hair loss and overall health. Blood tests may be necessary to check for nutritional deficiencies and adjust supplementation as needed.

In conclusion, hair loss after bariatric surgery can be a complex issue influenced by various factors. It is essential to address nutritional deficiencies, consider appropriate medical treatments, and adopt gentle hair care practices. Consulting with healthcare professionals, including dietitians and dermatologists, can provide tailored strategies to manage and potentially reverse hair loss. If you are experiencing significant hair loss, do not hesitate to seek professional advice to explore the best options for your situation.
