Can Du Shiping and Rifampicin be taken together?
The otolaryngologist prescribed me Rifampicin 0.5 mg for the treatment of tinnitus to be taken before bedtime.
I have been taking it for nearly 9 months and recently developed a shingles rash.
The dermatologist prescribed me Doxepin to take before bedtime.
Can I take these two medications together?
eagle, 50~59 year old female. Ask Date: 2020/09/01
Dr. Chen Bozhang reply Family Medicine
Hello Mr.
Eagle: Both Lyrica (Pregabalin) and Duloxetine have anxiolytic effects and can aid in sleep.
Using them together may enhance the sedative effect; this might be acceptable for patients with long-term severe sleep disorders, but for individuals without sleep issues, it could lead to prolonged sleep, fatigue, or difficulties with concentration due to additive effects.
It is recommended not to use them concurrently, or to consult the primary care physician to reassess the necessity of using these medications.

When considering the concurrent use of Doxepin and Rifampin, it is essential to understand the pharmacological properties of both medications and their potential interactions. Doxepin is a tricyclic antidepressant (TCA) that is often prescribed for its sedative properties and is used to treat conditions such as depression, anxiety, and insomnia. Rifampin, on the other hand, is an antibiotic primarily used to treat tuberculosis and certain bacterial infections. It is known for its ability to induce liver enzymes, particularly cytochrome P450 enzymes, which can significantly affect the metabolism of various drugs.
The primary concern when combining Doxepin with Rifampin is the potential for reduced efficacy of Doxepin. Rifampin is a potent inducer of liver enzymes, which means it can increase the metabolism of Doxepin, leading to lower plasma levels of the drug. This reduction in Doxepin levels may result in decreased therapeutic effects, potentially exacerbating the symptoms for which Doxepin was prescribed, such as insomnia or anxiety.
Moreover, Doxepin has a relatively narrow therapeutic index, meaning that the difference between an effective dose and a toxic dose is small. Therefore, any significant alteration in its metabolism could lead to either subtherapeutic effects or an increased risk of side effects if the dose is not adjusted accordingly.
In addition to the pharmacokinetic interactions, both medications can have overlapping side effects, such as sedation and dizziness. When taken together, these effects may be amplified, leading to increased drowsiness or impaired cognitive function, which could be particularly concerning if you need to perform tasks that require full alertness.
Given these considerations, it is crucial to consult with your healthcare provider before taking Doxepin and Rifampin together. Your doctor may recommend monitoring your response to Doxepin more closely or adjusting the dosage to ensure that you receive the intended therapeutic benefits without experiencing adverse effects. Additionally, they may explore alternative treatments for your condition that do not pose the same risk of interaction.
In summary, while it may be possible to take Doxepin and Rifampin together, it is not without risks. The potential for reduced efficacy of Doxepin due to the enzyme-inducing effects of Rifampin, along with the possibility of increased sedation, necessitates careful consideration and professional guidance. Always communicate openly with your healthcare provider about all medications you are taking, including over-the-counter drugs and supplements, to ensure safe and effective treatment.
