Menstrual issues
Hello, Doctor.
I underwent a medical abortion on July 23, and I stopped bleeding on August 12 (I had a follow-up appointment on August 10 and had protected intercourse).
Starting September 5, I noticed mixed yellow-brown discharge.
After my weight training on September 6, the discharge turned dark brown with a blood-like odor, but it was minimal.
On September 8, it changed to bright red with blood clots, and the amount was similar to a menstrual period.
Is this a sign of my normal menstrual cycle returning, or could it be incomplete bleeding from the medical abortion? (During my follow-up on August 10, I was still bleeding, but the doctor prescribed three days of uterine contraction medication and said that if I stopped bleeding after finishing the medication, I wouldn't need to return for another appointment.) Additionally, I took a pregnancy test on August 26, which showed one line.
Should I test again? Thank you, Doctor.

Post-abortion bleeding can be a complex issue, and understanding whether the bleeding you are experiencing is a normal part of recovery or a sign of complications is crucial. Based on your timeline and symptoms, it seems you are navigating a common post-abortion experience, but there are several factors to consider.
Firstly, after a medical abortion (or "medication abortion"), it is typical for bleeding to occur as your body expels the pregnancy tissue. This bleeding can vary in duration and intensity, often resembling a menstrual period. The fact that you experienced bleeding until August 12, approximately three weeks after your abortion, is within the normal range. It is also common for the bleeding to change in color and consistency, transitioning from bright red to brown or dark red as the body clears out the remaining tissue.
The mixed yellow-brown discharge you noted on September 5 could be part of the normal healing process. However, the subsequent change to dark brown discharge with a blood-like odor after heavy exercise raises some concerns. Physical activity can sometimes exacerbate bleeding, especially if there is still residual tissue in the uterus. The presence of fresh blood and blood clots on September 8, which you described as similar in volume to your menstrual flow, suggests that this could be more than just the normal post-abortion bleeding.
It is essential to differentiate between normal post-abortion bleeding and potential complications such as retained products of conception (RPOC). RPOC can occur if some tissue remains in the uterus after the abortion, leading to continued bleeding and possibly infection. The fact that you had a follow-up appointment on August 10, during which you were prescribed medication to help with uterine contractions, indicates that your healthcare provider was monitoring your recovery closely. If you were still experiencing bleeding at that time, it could suggest that your body was still in the process of expelling tissue.
Regarding your pregnancy test on August 26 showing one line, this indicates that you were likely no longer pregnant at that time. However, if you are still experiencing significant bleeding or if the bleeding continues to be heavy or accompanied by severe pain, it would be prudent to consult your healthcare provider again. They may recommend an ultrasound to check for any retained tissue or other complications.
In summary, while some bleeding and discharge after a medical abortion is normal, the changes you are experiencing—especially the return of bright red blood and clots—should be evaluated by a healthcare professional. It is always better to err on the side of caution when it comes to your reproductive health. If you have any concerns about the amount of bleeding, the presence of clots, or any other symptoms such as fever or severe pain, please seek medical attention promptly. Your health and peace of mind are paramount, and your healthcare provider can offer the best guidance based on your specific situation.
