Redness and peeling in the anal and groin area?
Hello, Director Ke.
Previously, I was advised by the outpatient physician to use medication and suppositories for two months for my thrombosed external hemorrhoid.
Since the external hemorrhoid has resolved, I stopped the medication about a month after starting it.
However, I noticed peeling in the groin area above the anus (which is not painful).
After about a week of peeling, today I am experiencing redness, swelling, and itching.
I am wondering if this is related to frequent washing, soaking, and using the hemorrhoid cream.
Can this heal on its own, or do I need to return for a follow-up appointment? Attached is a picture: https://upload.cc/i1/2020/09/20/NJcApC.jpg Thank you for your assistance, doctor.

It seems that excessive exposure to water has caused skin inflammation.
I recommend using a topical ointment for now; it should recover in about 3 to 5 days.

Understanding skin peeling and redness in the anal and groin area can be quite complex, as various factors can contribute to these symptoms. Based on your description, it seems that you have experienced skin peeling followed by redness and itching in the area above the anus and in the groin region. This could be related to several potential causes, including skin irritation, fungal infections, or even a reaction to medications or topical treatments.
Firstly, frequent washing or soaking in water can lead to skin irritation, especially if the skin is sensitive or if harsh soaps are used. Over-cleansing can strip the skin of its natural oils, leading to dryness and peeling. Additionally, if you have been using topical treatments like hemorrhoid creams, these can sometimes cause irritation or allergic reactions, particularly if used excessively or if the skin is already compromised.
The symptoms you describe—peeling skin, redness, and itching—could also suggest a fungal infection, such as tinea cruris (jock itch), which is common in warm, moist areas of the body. This condition typically presents with red, itchy patches that may peel or flake. It is important to note that fungal infections can thrive in areas that are frequently damp, so maintaining dryness is crucial.
In terms of treatment, if the symptoms are mild and you do not have any signs of infection (such as significant swelling, pus, or fever), you might consider allowing the area to heal on its own. However, it is essential to keep the area clean and dry. You can use a gentle, fragrance-free cleanser and avoid any harsh soaps or scrubs. After washing, ensure that the area is thoroughly dried, as moisture can exacerbate irritation and promote fungal growth.
If the redness and itching persist or worsen, or if you notice any signs of infection, it would be wise to consult a healthcare professional. A dermatologist or a proctologist can provide a thorough examination and may recommend antifungal treatments or topical steroids to reduce inflammation and itching. They can also rule out other potential causes, such as contact dermatitis or psoriasis, which may require different treatment approaches.
In summary, while some cases of skin peeling and redness in the anal and groin area can resolve on their own with proper care, it is crucial to monitor your symptoms closely. If you experience any worsening of your condition or if you have concerns about the nature of the skin changes, seeking medical advice is the best course of action. Remember, maintaining good hygiene while avoiding over-cleansing is key to preventing further irritation in sensitive areas.
