Hypothyroidism and Palpitations
Hello Dr.
Lee, I would like to ask you a question.
Last year, I underwent an advanced health check-up and was diagnosed with hypothyroidism.
I am currently taking one tablet of Eltroxin 50 mcg.
I often experience severe palpitations and sometimes have difficulty breathing.
Can hypothyroidism cause palpitations? Or could the medication for treating my thyroid condition be affecting my palpitations? Additionally, I frequently feel extremely fatigued.
Lastly, after treatment for hypothyroidism, will I be able to stop the medication, or will I need to take it for life? Thank you in advance for your response, Dr.
Lee.

Dr. Li Jian reply Internal Medicine
Palpitations are often a symptom of hyperthyroidism.
When taking thyroid hormone, thyroid function should be monitored every three months to determine if it is normal and whether lifelong medication is necessary.
Regular blood tests are required to assess this.

More Info
Hypothyroidism, or an underactive thyroid, can indeed lead to a variety of symptoms, including heart palpitations and feelings of breathlessness. The thyroid gland plays a crucial role in regulating metabolism, and when it is not producing enough thyroid hormones, it can affect various bodily functions, including heart rate and rhythm.
Heart Palpitations and Hypothyroidism
Heart palpitations are sensations of a racing, fluttering, or pounding heart. In hypothyroidism, the body’s metabolism slows down, which can lead to a decrease in heart rate (bradycardia). However, some individuals may experience palpitations due to fluctuations in thyroid hormone levels or as a response to stress and anxiety, which can be common in those with chronic health conditions.
When you are on medication like Eltroxin (levothyroxine), which is a synthetic form of the thyroid hormone thyroxine (T4), it is intended to normalize thyroid hormone levels in your body. If the dosage is too high, it can lead to symptoms of hyperthyroidism, including palpitations, anxiety, and increased heart rate. Conversely, if the dosage is too low, you may continue to experience symptoms related to hypothyroidism, such as fatigue and possibly palpitations.
Treatment and Management
Regarding your treatment with Eltroxin, it is essential to have regular follow-ups with your healthcare provider to monitor your thyroid hormone levels through blood tests (typically TSH and free T4). This will help ensure that you are on the correct dosage. If you are experiencing significant heart palpitations, it is crucial to discuss this with your doctor, as they may need to adjust your medication or investigate other potential causes.
Fatigue and Hypothyroidism
Fatigue is another common symptom of hypothyroidism. The lack of adequate thyroid hormone can lead to decreased energy levels and a general feeling of tiredness. As your thyroid hormone levels normalize with appropriate treatment, you should start to notice an improvement in your energy levels. However, it may take some time for your body to adjust to the medication, and you may need to be patient as you monitor your symptoms.
Long-term Management of Hypothyroidism
As for whether hypothyroidism can be "cured" or if you will need to take medication for life, it largely depends on the underlying cause of your hypothyroidism. In cases where hypothyroidism is due to autoimmune conditions like Hashimoto's thyroiditis, lifelong treatment is typically necessary. However, if the hypothyroidism is caused by a temporary condition or certain medications, it may resolve over time.
Regular monitoring and communication with your healthcare provider are key. They will guide you on the necessity of ongoing treatment and any lifestyle modifications that may help manage your symptoms better.
Conclusion
In summary, hypothyroidism can indeed cause heart palpitations, and the treatment with Eltroxin should help alleviate many of your symptoms, including fatigue. However, it is vital to ensure that your medication dosage is appropriate and to discuss any ongoing symptoms, such as palpitations and breathlessness, with your healthcare provider. Regular follow-ups and blood tests will help manage your condition effectively, and while many individuals with hypothyroidism require lifelong treatment, the specifics can vary based on individual circumstances. Always consult with your doctor for personalized advice and treatment options.
