Dull pain under the left rib cage?

I apologize for bothering you, Doctor.
I'm not sure if this question has been asked before.
Back in June, I suddenly experienced persistent discomfort and pain, which I initially thought was a stomach issue.
I searched online but couldn't find similar cases.
I visited a family medicine doctor and after taking medication, I did see significant improvement; the pain changed from constant to occasional and not time-specific.
At first, I suspected it might be psychological or just a figment of my imagination.
Later, I considered my long-standing constipation issue (I typically have a bowel movement about once a week or less), so I returned for a follow-up.
I am now regularly visiting a gastroenterologist and taking medication for constipation.
An ultrasound showed only fecal retention and bloating.
As my constipation has gradually improved, I've noticed that the discomfort tends to occur the day after I haven't had a bowel movement.
When I am having regular bowel movements, it seems I don't experience much discomfort.
My question is, can resolving the constipation issue restore my health? Should I be concerned about the possibility of colorectal cancer? I find myself worrying excessively, and perhaps there are psychological factors involved.
I hope you can provide some advice on improving my gastrointestinal health.
Thank you for your help!

Dr. Hong Yuanbin reply Internal Medicine

Based on your description, it is indeed most likely that constipation is the cause.
It is advisable to maintain a bowel movement every 1-2 days.
Suitable improvement methods include increasing vegetable intake, exercising regularly, establishing a consistent routine, and developing a regular time for bowel movements.
If these methods do not yield satisfactory results, you may consider supplementing with probiotics or consuming fruits that promote bowel movements, such as dragon fruit.
Regarding the concern about colorectal cancer, the likelihood is very low unless you notice symptoms such as passing black stools, blood in the stool, or severe intestinal obstruction, in which case you should seek medical evaluation.
Thank you for your inquiry.

Understanding left rib pain, particularly in the context of gastrointestinal health, can be quite complex. Your symptoms, including the persistent dull pain and issues with constipation, suggest a multifaceted problem that may involve both physical and psychological factors.

Firstly, it’s important to recognize that left rib pain can arise from various sources. In your case, since you have a history of gastrointestinal issues, the pain may be related to your digestive system. Conditions such as gastritis, gastroesophageal reflux disease (GERD), or even issues related to the spleen or pancreas can manifest as pain in the left upper quadrant of the abdomen. Additionally, constipation can lead to discomfort and pain due to the buildup of stool in the intestines, which can create pressure and discomfort in the abdominal area.

Your mention of experiencing pain that correlates with your bowel movements is significant. It suggests that your gastrointestinal health is closely linked to your symptoms. When you are constipated, the pressure in your abdomen can increase, leading to discomfort or pain. Conversely, regular bowel movements can alleviate this pressure, resulting in a reduction of pain. This pattern indicates that addressing your constipation may indeed help in resolving your left rib pain.

Regarding your concern about colorectal cancer, it’s understandable to feel anxious, especially when experiencing persistent gastrointestinal symptoms. However, it’s important to note that while changes in bowel habits can be a symptom of colorectal cancer, they are more commonly associated with benign conditions such as irritable bowel syndrome (IBS) or functional constipation. Given your age and the fact that you have undergone imaging studies that did not reveal any alarming findings, the likelihood of colorectal cancer is relatively low. Nevertheless, if you have persistent concerns, it’s always a good idea to discuss them with your healthcare provider, who may recommend further screening or tests if necessary.

To improve your gastrointestinal health and alleviate your symptoms, consider the following recommendations:
1. Dietary Adjustments: Increase your intake of dietary fiber through fruits, vegetables, whole grains, and legumes. Fiber helps to bulk up stool and can promote regular bowel movements. Additionally, staying hydrated is crucial, as adequate fluid intake can help soften stool.

2. Regular Exercise: Physical activity can stimulate bowel function and help alleviate constipation. Aim for at least 30 minutes of moderate exercise most days of the week.

3. Establish a Routine: Try to create a regular schedule for bowel movements. This can help train your body to have more consistent bowel habits.

4. Stress Management: Since you mentioned the possibility of psychological factors contributing to your symptoms, consider incorporating stress-reducing activities into your routine, such as yoga, meditation, or deep-breathing exercises.

5. Medication Review: If you are taking medications for constipation, discuss with your doctor whether they are the most appropriate for your situation. Sometimes, a change in medication can lead to better outcomes.

6. Follow-Up Care: Continue your follow-up appointments with your gastroenterologist. If your symptoms persist or worsen, further evaluation may be warranted, including additional imaging or endoscopic procedures.

In conclusion, while your left rib pain may be concerning, it appears to be closely tied to your gastrointestinal health, particularly your constipation. By addressing the underlying issues related to your bowel habits and maintaining open communication with your healthcare provider, you can work towards improving your overall gastrointestinal health and alleviating your symptoms. Remember, it’s essential to listen to your body and seek medical advice whenever you feel uncertain about your health.
