Swelling and pain in the area of the mandible and cheekbone?
Hello Doctor, I have been experiencing pain in my lower right jaw (the area below my cheekbone feels like there is a rounded prominence when touched).
Additionally, the right side feels more prominent compared to the left.
I would like to know what could be causing this issue.
Since the location is somewhat awkward, which specialty should I consult if I visit a clinic? Thank you.

Dear Ms.
Chen: Regarding the lesion in the right mandible, possible causes may include folliculitis, sebaceous gland inflammation, cellulitis, lymphadenitis, mandibular hyperplasia or dysplastic changes, lymphatic tuberculosis, metastatic cancer in the head and neck, lymphoma, among others.
Clinicians must assess the symptoms and the patient's subjective experiences; if necessary, blood tests, imaging studies, and pathological biopsies may be required for diagnosis.
Please discuss and address this with your family physician first, or you may directly visit an ENT outpatient clinic at a regional hospital or higher for further evaluation.

Understanding pain and swelling in the lower jaw area can be quite complex, as there are multiple potential causes that could lead to the symptoms you are experiencing. The area you described, located beneath the cheekbone and near the jaw, could be affected by various conditions, including dental issues, temporomandibular joint (TMJ) disorders, infections, or even systemic conditions.
1. Dental Issues: One of the most common causes of pain and swelling in the lower jaw area is dental problems. This could include tooth decay, abscesses, or gum disease. If there is an infection in a tooth or the surrounding gum tissue, it can lead to localized swelling and pain. A dental examination, including X-rays, may be necessary to rule out these issues.
2. Temporomandibular Joint Disorders (TMJ): The TMJ connects your jawbone to your skull and can become inflamed or dysfunctional due to various reasons, including teeth grinding, jaw clenching, or arthritis. Symptoms of TMJ disorders often include pain in the jaw, difficulty chewing, and swelling around the joint area. If you suspect this might be the case, a consultation with a dentist or an oral surgeon specializing in TMJ disorders would be appropriate.
3. Infections: Infections in the jaw area, such as osteomyelitis (infection of the bone) or cellulitis (a bacterial skin infection), can also cause swelling and pain. If you have any accompanying symptoms like fever, chills, or redness in the area, it is crucial to seek medical attention promptly.
4. Cysts or Tumors: Although less common, cysts or benign tumors can develop in the jaw area and may present as a palpable lump or swelling. If the swelling is persistent and does not resolve, imaging studies such as a CT scan or MRI may be warranted to evaluate the underlying structures.
5. Systemic Conditions: Conditions such as rheumatoid arthritis or other inflammatory diseases can also affect the jaw area, leading to pain and swelling. If you have a history of such conditions, it would be worth discussing with your healthcare provider.
Given the location and nature of your symptoms, I would recommend starting with a visit to a dentist, as they can evaluate for any dental-related issues. If dental problems are ruled out, you may then consider seeing an oral surgeon or a specialist in TMJ disorders. If you suspect an infection or if the swelling is significant, it may be advisable to seek immediate medical attention, potentially from an urgent care facility or emergency room.
In summary, the pain and swelling in your lower jaw could stem from various causes, including dental issues, TMJ disorders, infections, or even systemic conditions. A thorough examination by a dental professional is a crucial first step in determining the underlying cause and appropriate treatment. If necessary, they can refer you to the appropriate specialist for further evaluation and management.
