Inquiry about Epididymitis

Hello Doctor: Recently, I had an infection of the parotid gland, and afterwards I experienced discomfort in my lower region.
I visited two clinics, and both doctors diagnosed me with epididymitis (based only on physical examination), so they prescribed pain relievers.
It has been a week now, and although the pain has decreased, there is still swelling.
Is this normal? Thank you~

Niming, 10~19 year old female. Ask Date: 2020/10/18

Dr. Du Shixi reply Urology

Hello: It is possible, but most of the time the swelling will subside completely within a month, or it may recur (swelling may return), in which case medical attention is needed.

More Info

Epididymitis is an inflammation of the epididymis, a tube located at the back of the testicle that stores and carries sperm. This condition can be caused by various factors, including bacterial infections, sexually transmitted infections (STIs), or even non-infectious causes such as trauma or certain medications. Understanding the symptoms, treatment options, and recovery time is crucial for managing this condition effectively.

Symptoms of Epididymitis
The primary symptoms of epididymitis include:
- Pain and tenderness in the affected testicle, which may radiate to the groin or lower abdomen.

- Swelling of the epididymis, which can lead to noticeable enlargement of the testicle.

- Redness or warmth over the affected area.

- Discharge from the penis, especially if the cause is an STI.

- Pain during urination or ejaculation.

- Fever or chills, indicating a more severe infection.

In your case, it sounds like you have experienced some of these symptoms, particularly pain and swelling. The fact that the pain has decreased but swelling persists after a week is not uncommon. Epididymitis can take time to resolve, and swelling may linger even after the acute pain subsides.

Treatment Options
Treatment for epididymitis typically involves:
1. Antibiotics: If the condition is caused by a bacterial infection, antibiotics are the primary treatment. The choice of antibiotic may depend on the suspected cause (e.g., STIs, urinary tract infections).

2. Pain Relief: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and reduce inflammation.

3. Rest and Support: Elevating the scrotum and wearing supportive underwear can alleviate discomfort.

4. Ice Packs: Applying ice packs to the affected area can help reduce swelling and pain.

5. Avoiding Sexual Activity: Until the infection is fully treated, it’s advisable to avoid sexual intercourse to prevent further irritation or transmission of STIs.

Recovery Time
Recovery time for epididymitis can vary based on the underlying cause and the treatment provided. Generally, with appropriate antibiotic treatment, symptoms may start to improve within a few days, but complete resolution, including any swelling, may take several weeks. It’s essential to follow up with your healthcare provider if symptoms persist or worsen, as this could indicate complications or the need for a different treatment approach.

When to Seek Further Medical Attention
You should consider seeking further medical attention if:
- The swelling does not improve after a week or worsens.

- You experience severe pain that is not relieved by over-the-counter medications.

- You develop a fever or chills.

- There is a significant change in the discharge (e.g., increased volume, change in color).

- You have any concerns about your symptoms or treatment.

In conclusion, while it is normal for swelling to persist after the pain has decreased, it is essential to monitor your symptoms closely. If you have any concerns or if the swelling does not improve, please consult your healthcare provider for further evaluation and management. They may consider additional diagnostic tests, such as an ultrasound, to rule out other conditions like testicular torsion or abscess formation.
