After hemorrhoid surgery, there is a persistent feeling of needing to have a bowel movement?
Hello, I have grade 4 hemorrhoids and underwent a circular surgery on September 18th.
It has been over a month now; the wound around the anal area has healed, but I have been using anti-inflammatory suppositories.
If I skip a day without using them, I feel the urge to have a bowel movement.
Is it possible that the surgical site is still inflamed? Should I continue using the suppositories? I also need to go to the bathroom two to three times in the morning; is that normal? Thank you, Xiao Lin.

Although the wound from hemorrhoid circular excision surgery heals, the scar tissue that forms after healing, along with the presence of sutures, may lead to a foreign body sensation in the short term.
This is expected to improve over time.
However, if the condition persists without improvement, it is recommended to return to the original treating physician for follow-up examination.

Post-hemorrhoid surgery, particularly after a procedure for grade IV hemorrhoids, can lead to various sensations and complications during the recovery phase. It is not uncommon for patients to experience a persistent urge to defecate even after the surgical site has healed. This sensation can be attributed to several factors, including inflammation, nerve sensitivity, and changes in bowel habits following surgery.
Firstly, the sensation of needing to defecate can be a result of residual inflammation at the surgical site. Even if the external wounds appear to have healed, the internal tissues may still be recovering. The use of topical medications, such as anti-inflammatory suppositories, is often recommended to help reduce inflammation and promote healing. If you have been using these medications consistently and still feel an urge to defecate, it may be beneficial to consult your surgeon or healthcare provider. They can assess whether the inflammation is still present and if continued use of the suppositories is necessary.
Secondly, after surgery, the bowel habits of patients can change significantly. Some individuals may experience increased frequency of bowel movements due to altered bowel function or anxiety about having a bowel movement after surgery. This can lead to a psychological component where the fear of pain or discomfort during defecation causes a heightened awareness of bowel sensations. It is essential to maintain a healthy diet rich in fiber and stay hydrated to promote regular bowel movements and reduce the urge to strain.
Additionally, the surgical site may have altered nerve sensitivity, which can lead to a heightened sensation of urgency. This is often temporary and should improve as the nerves heal and adapt post-surgery. However, if the sensation persists or worsens, it is crucial to discuss this with your healthcare provider, as they may want to rule out any complications such as anal fissures or strictures that could be contributing to your symptoms.
Regarding the frequency of bowel movements, it is not unusual for patients to experience changes in their bowel habits after hemorrhoid surgery. Some may find themselves needing to go multiple times a day, especially if they are consuming a high-fiber diet or taking stool softeners. As long as the bowel movements are not accompanied by pain, bleeding, or significant discomfort, this may be a normal part of your recovery process.
In summary, while it is common to experience a persistent urge to defecate after hemorrhoid surgery, it is essential to monitor your symptoms closely. If the urge continues or is accompanied by other concerning symptoms, please reach out to your healthcare provider for further evaluation. They can provide personalized advice based on your recovery progress and may suggest adjustments to your treatment plan, including the use of suppositories or other medications. Remember, recovery can take time, and it is crucial to be patient with your body as it heals.
