Anal itching

Hello doctor, I have been experiencing itching in my anus for 1-2 years.
I can feel a soft, fleshy lump below the external anal area.
It tends to itch particularly at night or after a bowel movement.
Sometimes, using hemorrhoid cream or antifungal cream provides relief.
Could this be an external hemorrhoid?

Tian, 30~39 year old female. Ask Date: 2020/11/09

Dr. Ke Fangxu reply Surgery

Anal itching may be caused by excessive wiping.
In addition to medication and topical treatments, there are three important considerations: 1.
Do not scratch to avoid causing an infection in the wounds.
2.
Temporarily avoid spicy foods to prevent capsaicin from irritating the skin.
3.
Avoid using soap or body wash to clean the anal area to prevent excessive removal of the skin's natural oils.
If these measures are followed, improvement can generally be seen in about a week.
If there is no improvement, it is important to consult a physician for evaluation and prescription medication.
Antifungal medications are typically used for tinea infections and are not usually prescribed for anal itching.
If there is a lump, it is more likely to be an external hemorrhoid, but a physician's examination is necessary for a definitive diagnosis.

More Info

Anal itching, also known as pruritus ani, is a common condition that can be caused by a variety of factors. Understanding the underlying causes and available treatment options is essential for effective management.

Causes of Anal Itching
1. Hemorrhoids: One of the most common causes of anal itching is hemorrhoids, which are swollen veins in the lower rectum and anus. They can be internal or external, and symptoms may include itching, discomfort, and sometimes bleeding. The soft, fleshy lump you described could be indicative of an external hemorrhoid.

2. Skin Conditions: Conditions such as eczema, psoriasis, or fungal infections (like tinea cruris, also known as jock itch) can lead to itching around the anal area. These conditions may cause inflammation and irritation, contributing to discomfort.

3. Hygiene Issues: Both inadequate and excessive hygiene can lead to anal itching. Insufficient cleaning after bowel movements can leave irritants on the skin, while excessive washing or using harsh soaps can strip the skin of its natural oils, leading to dryness and irritation.

4. Dietary Factors: Certain foods and beverages, such as caffeine, alcohol, spicy foods, and citrus fruits, can irritate the anal area and lead to itching.
5. Infections: Anal itching can also be caused by infections, including sexually transmitted infections (STIs) or pinworms, particularly in children.

6. Allergic Reactions: Allergies to soaps, toilet paper, or laundry detergents can cause localized irritation and itching.

Treatment Options
1. Over-the-Counter Medications: You mentioned using hemorrhoid cream, which can provide temporary relief from itching and discomfort associated with hemorrhoids. Antifungal creams may help if a fungal infection is suspected.

2. Proper Hygiene: Maintaining good hygiene is crucial. Gently clean the anal area with mild soap and water after bowel movements, and ensure it is thoroughly dried. Avoid using scented products that can irritate the skin.

3. Dietary Changes: Keeping a food diary to identify potential dietary triggers can be helpful. Reducing or eliminating spicy foods, caffeine, and alcohol may alleviate symptoms.

4. Topical Treatments: In addition to hemorrhoid creams, hydrocortisone cream can reduce inflammation and itching. However, it should be used sparingly and not for prolonged periods without medical advice.

5. Avoiding Irritants: Switching to unscented toilet paper and avoiding harsh soaps can help minimize irritation.

6. Consultation with a Healthcare Provider: If symptoms persist despite home treatment, it is important to consult a healthcare provider. They can perform a physical examination to determine the exact cause of the itching and recommend appropriate treatments, which may include prescription medications or procedures if necessary.

Conclusion
Anal itching can be a frustrating and uncomfortable condition, but understanding its causes and treatment options can lead to effective management. Given that your symptoms have persisted for 1-2 years, it is advisable to seek medical attention to rule out any underlying conditions, such as hemorrhoids or skin disorders. A healthcare provider can provide a tailored treatment plan based on your specific situation, ensuring that you receive the appropriate care for your symptoms.
