Right eye pain?

Hello, doctor.
I have the following concerns:
1.
My right eye has a prescription of -700 diopters and I experience floaters.
For the past four days, I have been experiencing pain in my right eye and orbit, which is more intense when I look towards the 3 o'clock and 9 o'clock positions.
What could be the cause of this?
2.
In the lower left corner of my right eye's visual field, I notice a phenomenon where it appears to be obscured by a gray veil.
3.
When I close my eyes before sleeping, instead of darkness, I see an orange light with cracks in the image, which disappears after a few seconds.
4.
I had an examination of my optic nerve, and the doctor mentioned that there is some swelling of the optic nerve.
What should I do to help myself? Thank you, doctor.

Dear elderly lady: Based on your description, it is difficult to determine the cause of your pain.
Regarding the visual field loss, I recommend that you visit a large hospital for further examination.
As for the phenomenon of visual persistence when closing your eyes, it likely does not have any significant relation.
It could be related to optic nerve edema? Again, I suggest you have a thorough examination at a large hospital.

Hello! Thank you for your detailed description of your symptoms. Based on what you've shared, it seems that you are experiencing a combination of right eye pain, visual disturbances, and possibly some underlying issues with your optic nerve. Let's break down your concerns and discuss potential causes and self-care strategies.

1. Right Eye Pain: The pain you are experiencing in your right eye, particularly when moving your gaze to the 3 o'clock and 9 o'clock positions, could be indicative of several conditions. One possibility is that you may have inflammation or irritation in the eye, which could be due to conditions like uveitis or scleritis. Another potential cause could be related to the muscles around the eye, which can become strained or inflamed, especially if you have been using screens for extended periods. Given your history of myopia (700 degrees) and the presence of floaters, it's essential to consider that these symptoms could also be related to retinal issues.

2. Visual Disturbances: The phenomenon of seeing a gray veil in your lower right visual field could be a sign of retinal detachment or a vitreous hemorrhage, especially since you mentioned having floaters. The orange light and crack-like patterns you see when closing your eyes could be related to visual migraines or other neurological issues, particularly if they are transient. However, these symptoms warrant immediate attention, especially given your recent diagnosis of optic nerve swelling.

3. Optic Nerve Swelling: The swelling of the optic nerve (papilledema) can be caused by increased intracranial pressure, which can arise from various conditions, including migraines, tumors, or other neurological disorders. This is a serious condition that requires careful monitoring and possibly further investigation by a specialist.

Self-Care Tips:
While it's crucial to seek professional medical advice, here are some self-care strategies you can consider in the meantime:
- Rest Your Eyes: If you have been using screens frequently, take regular breaks using the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds. This can help reduce eye strain.

- Warm Compresses: Applying a warm compress to your eyes can help alleviate discomfort and promote relaxation of the eye muscles.

- Stay Hydrated: Ensure you are drinking enough water throughout the day, as dehydration can contribute to eye discomfort.

- Manage Stress: Stress can exacerbate eye pain and visual disturbances. Techniques such as mindfulness, meditation, or gentle yoga can help reduce stress levels.

- Monitor Symptoms: Keep a detailed log of your symptoms, including when they occur, their intensity, and any triggers you notice. This information can be invaluable for your healthcare provider.

When to Seek Immediate Care:
Given your symptoms, especially the visual disturbances and optic nerve swelling, it is crucial to seek medical attention as soon as possible. If you experience any sudden changes in vision, worsening pain, or new symptoms such as headaches or nausea, do not hesitate to go to the emergency room or an eye specialist.

In summary, while some self-care measures can help alleviate discomfort, your symptoms indicate that a thorough evaluation by an eye care professional is necessary. They can provide a comprehensive examination and determine the best course of action for your eye health. Please prioritize getting the care you need. Wishing you a speedy recovery!
