Sudden wheezing sounds during sleep?

Hello Dr.
Hong, I apologize for interrupting you during your busy schedule.
Recently, I have noticed that I experience wheezing sounds while sleeping.
After doing some research on Google, I suspect it might be related to respiratory issues, so I am reaching out to inquire.
The episodes are irregular, occurring approximately every few minutes, and I often wake up due to this sound.
Additionally, I have been experiencing frequent chest tightness and severe nasal congestion.
I measured my blood pressure, which seems normal, but my heart rate has reached 90 beats per minute.
I would like to know what possible issues could be causing these symptoms.

Huang, 20~29 year old female. Ask Date: 2020/12/12

Dr. Hong Yuanbin reply Internal Medicine

Hello, based on your description, it seems that temperature changes or allergies may have led to allergic rhinitis and bronchitis.
I recommend starting with keeping warm and wearing a mask, as well as engaging in moderate exercise, which can be helpful.
If symptoms persist, it may be necessary to visit an otolaryngologist and consider using some antihistamine medications.
Thank you for your inquiry.

Hello! Thank you for reaching out with your concerns regarding nocturnal wheezing and its potential impact on your sleep health. Based on the symptoms you've described—wheezing during sleep, frequent awakenings due to this sound, chest tightness, and nasal congestion—there are several potential underlying causes that we can explore.

Nocturnal wheezing is often associated with respiratory conditions, particularly asthma or other forms of reactive airway disease. Asthma can cause the airways to become inflamed and constricted, leading to wheezing, coughing, and difficulty breathing, especially at night when lying down can exacerbate symptoms. The fact that you are experiencing these symptoms intermittently and that they disrupt your sleep suggests that it may be worth investigating further.

Nasal congestion, which you mentioned is severe, can also contribute to breathing difficulties during sleep. When nasal passages are blocked, individuals often resort to mouth breathing, which can lead to a dry throat and exacerbate wheezing. Allergies, sinus infections, or even environmental factors like dust mites or pet dander could be contributing to both your nasal congestion and wheezing.
Additionally, the elevated heart rate you noted (90 beats per minute) could be a response to the stress your body is experiencing due to difficulty breathing. When the body perceives a lack of oxygen or experiences discomfort, it can trigger a fight-or-flight response, leading to an increased heart rate.
Given these symptoms, it is crucial to consider a few steps for further evaluation and management:
1. Consult a Healthcare Provider: It is essential to see a healthcare professional, preferably a pulmonologist or an allergist, who can perform a thorough evaluation. They may conduct tests such as spirometry to assess lung function, allergy testing, or even a sleep study to monitor your breathing patterns during sleep.

2. Environmental Modifications: Consider assessing your sleeping environment for potential allergens. This could include using hypoallergenic bedding, ensuring good ventilation, and minimizing exposure to dust and pet dander.
3. Medication Review: If you are currently taking any medications, review them with your healthcare provider to ensure they are not contributing to your symptoms. Additionally, if asthma or allergies are diagnosed, appropriate medications such as inhalers or antihistamines may be prescribed.

4. Lifestyle Changes: Incorporating regular physical activity, maintaining a healthy diet, and managing stress can significantly improve overall respiratory health. Avoiding smoking and limiting exposure to secondhand smoke is also crucial.

5. Sleep Hygiene: Establishing a consistent sleep routine and creating a comfortable sleep environment can help improve sleep quality. This includes keeping the bedroom dark, quiet, and at a comfortable temperature.

In summary, your symptoms of nocturnal wheezing, chest tightness, and nasal congestion warrant further investigation to determine the underlying cause. By consulting with a healthcare provider and making some lifestyle adjustments, you can work towards improving your sleep health and overall well-being. Please take care, and I hope you find relief soon!
