Issues After Artificial Insemination and Embryo Transfer

Hello Dr.
Tsao: I would like to ask, today is the 9th day after my implantation.
The symptoms I've experienced over the past few days include abdominal bloating, occasional unilateral breast tenderness, and brief sharp pain near my navel.
I've been having insomnia at night and only feel sleepy in the morning.
Today, shortly after waking up, I felt a dull ache in the lower middle part of my abdomen, similar to premenstrual symptoms.
Is this normal? This time, I had a mature egg on the right side, while there was no egg development on the left side.
However, I occasionally feel a dull ache on the left side of my abdomen near the fallopian tube after implantation.
What could be the reason for this? My basal body temperature has always been on the lower side (35-36°C), and today it barely reached 36°C.
I read online that early pregnancy symptoms can include a noticeable change in urine odor.
Does a low basal body temperature and lack of urine odor really indicate that I am not pregnant?

Qiqi, 30~39 year old female. Ask Date: 2020/12/18

Dr. Cao Guotao reply Obstetrics and Gynecology

After artificial insemination, it is common for individuals to experience a range of symptoms due to hormonal changes and the body’s response to the implantation process. In your case, you are reporting several symptoms that can be associated with early pregnancy or the effects of the hormonal medications used during the insemination process.

Firstly, the abdominal bloating and occasional breast tenderness you are experiencing can be attributed to the hormonal changes that occur after implantation. These symptoms are often similar to those experienced during the luteal phase of the menstrual cycle, where progesterone levels rise to support a potential pregnancy. The discomfort near your navel and the sensation of heaviness in your lower abdomen may also be related to these hormonal fluctuations or the physical changes occurring in your body as it prepares for a possible pregnancy.

The fact that you are experiencing a sensation similar to premenstrual symptoms, such as cramping or a dull ache in the lower abdomen, is also not uncommon. This can be due to the uterus beginning to prepare for implantation or, in some cases, the body reacting to the hormonal treatments. It is important to note that while these symptoms can indicate pregnancy, they can also be signs of the body adjusting to the changes brought on by the insemination process.

Regarding your concerns about the left side of your abdomen, where you feel discomfort near the fallopian tube, this could be due to several factors. If the right ovary had a mature follicle that was stimulated for ovulation, it is possible that the left side is experiencing referred pain or discomfort due to the hormonal changes or the physical presence of the ovary itself. However, if the pain is severe or persistent, it would be advisable to consult your healthcare provider to rule out any complications, such as an ectopic pregnancy or ovarian hyperstimulation syndrome (OHSS), especially if you were undergoing fertility treatments.

As for your body temperature and urine odor, a lower oral temperature (35-36 degrees Celsius) is not typically indicative of pregnancy. Normal body temperature can vary, but it is generally around 36.1 to 37.2 degrees Celsius. If your temperature remains consistently low, it may be worth discussing with your doctor, as it could indicate other underlying issues. Additionally, changes in urine odor can be influenced by hydration levels, diet, and hormonal changes. While some women report a stronger urine odor in early pregnancy, the absence of this symptom does not definitively indicate that you are not pregnant.

In summary, the symptoms you are experiencing can be normal following artificial insemination, but they can also be indicative of other conditions. It is crucial to maintain open communication with your healthcare provider, who can offer personalized advice and conduct any necessary evaluations to ensure your health and well-being during this time. If your symptoms worsen or if you have any concerns, do not hesitate to seek medical attention.
