Bleeding during bowel movements after hemorrhoid ligation?
Hello Dr.
Yu: I underwent a hemorrhoid ligation surgery in mid-October.
After the examination, the doctor mentioned that there was only one hemorrhoid and that I did not need to return for a follow-up after the procedure.
Post-surgery, I haven't experienced much discomfort or pain; however, I notice fresh blood when I apply pressure to the anus with toilet paper after bowel movements (the bleeding stops immediately after).
This has continued to occur, and I can feel drops of blood from the anus if the bowel movement lasts longer than five minutes.
Is this a normal part of the recovery process (and will it gradually improve on its own), or should I be more proactive with medication (such as applying ointment internally) or sitz baths, or do I need to schedule a follow-up appointment?

After undergoing hemorrhoid ligation, experiencing some bleeding after bowel movements can be a common occurrence, especially in the days and weeks following the procedure. However, it is essential to differentiate between what is considered normal and what may require further medical attention.
In your case, the presence of bright red blood on the toilet paper after bowel movements, particularly if it stops shortly after, can be attributed to several factors related to the surgery. Hemorrhoid ligation involves placing a rubber band around the base of the hemorrhoid, which cuts off its blood supply. As the tissue dies and eventually falls off, it can lead to some bleeding. This bleeding is typically minimal and should resolve on its own as the area heals.
However, if you are experiencing persistent bleeding, especially if it is significant or accompanied by pain, it could indicate complications such as:
1. Infection: Surgical sites can become infected, leading to increased bleeding and discomfort.
2. Incomplete ligation: If the ligation was not entirely successful, the hemorrhoid may still be present and could bleed.
3. Anal fissures: Straining during bowel movements can lead to the development of fissures, which can cause bleeding.
4. Other gastrointestinal issues: Although less likely, other conditions could cause rectal bleeding, necessitating further evaluation.
To manage your symptoms and promote healing, consider the following recommendations:
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to soften stools and reduce straining during bowel movements. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying well-hydrated is also crucial.
2. Sitz Baths: Taking warm sitz baths can help soothe the anal area and promote blood flow, aiding in healing. Aim for 10-15 minutes several times a day, especially after bowel movements.
3. Topical Treatments: Over-the-counter hemorrhoid creams or ointments can provide relief from discomfort and may help reduce inflammation. However, consult your doctor before applying any medication, especially if you are considering using a medicated ointment internally.
4. Avoid Straining: If you feel the urge to have a bowel movement, try to go as soon as possible. Avoid sitting on the toilet for extended periods, as this can increase pressure on the anal area.
5. Follow-Up with Your Doctor: Since you are still experiencing bleeding after bowel movements, it is advisable to schedule a follow-up appointment with your healthcare provider. They can assess your condition, rule out any complications, and provide tailored advice based on your specific situation.
In summary, while some bleeding after hemorrhoid ligation can be normal, persistent or significant bleeding warrants further evaluation. Taking proactive steps to manage your symptoms and consulting with your healthcare provider will help ensure a smooth recovery.
