Urinary incontinence issues?

Hello, I recently had my first sexual experience and have noticed frequent urinary leakage issues and increased urgency to urinate (though I have been drinking a lot of water).
Even after just using the restroom, I sometimes find a few drops when I put on my pants.
This morning, I suddenly woke up and found a small amount of urine on my underwear, but there was no odor.
I'm only 22 years old; is this abnormal? Could it be related to my sexual activity?

L, 20~29 year old female. Ask Date: 2020/12/20

Dr. Sun Hongshun reply Urology

Hello, I recommend that you first undergo a urinalysis to rule out a urinary tract infection.
If this condition persists, a urodynamic study will need to be arranged.
This is certainly abnormal for someone who is 22 years old, and it may not necessarily be directly related to sexual activity, but you should seek medical attention as soon as possible.

It's understandable to feel concerned about urinary leakage, especially after your first sexual experience. While some degree of anxiety is normal, it's important to approach this issue with a clear understanding of what might be happening.

Firstly, urinary leakage, or incontinence, can occur for various reasons, and it is not uncommon for individuals to experience changes in urinary habits after engaging in sexual activity for the first time. The pelvic floor muscles, which support the bladder and help control urination, may be affected during sexual intercourse. If these muscles are tense or if there is any trauma (even minor) to the vaginal area, it can lead to temporary changes in urinary function, including leakage.

In your case, since you mentioned that you have been drinking a lot of water, increased fluid intake can naturally lead to more frequent urination. However, if you are experiencing leakage even when you feel you have just emptied your bladder, it may indicate that your pelvic floor muscles are not functioning optimally. This could be due to a variety of factors, including anxiety, tension, or even the physical changes that occur during your first sexual experience.

It's also worth noting that the first sexual encounter can be a mix of excitement and nervousness, which can lead to muscle tension. This tension can affect how well the pelvic floor muscles work, potentially leading to issues like urinary leakage. Additionally, if there was any discomfort or pain during intercourse, it could have caused you to unconsciously tighten these muscles, further complicating the situation.

If you are experiencing leakage that is persistent or worsening, it would be advisable to consult a healthcare professional. They can assess your symptoms more thoroughly and determine if there is an underlying issue that needs to be addressed. A gynecologist or a urologist would be best suited for this, as they can provide insights specific to your situation.

In the meantime, there are a few strategies you can employ to help manage your symptoms:
1. Pelvic Floor Exercises: Engaging in Kegel exercises can strengthen the pelvic floor muscles. These exercises involve contracting and relaxing the muscles you would use to stop urination. Regular practice can improve muscle tone and control.

2. Mindfulness and Relaxation Techniques: Since anxiety can contribute to muscle tension, practicing relaxation techniques such as deep breathing, yoga, or meditation may help alleviate some of the tension in your pelvic area.

3. Hydration Management: While staying hydrated is important, consider moderating your fluid intake, especially before engaging in activities where you might be concerned about leakage.

4. Avoiding Irritants: Certain foods and drinks, such as caffeine and alcohol, can irritate the bladder and increase urgency and frequency. Monitoring your diet may help reduce symptoms.

5. Consulting a Professional: If the issue persists, seeking professional help is crucial. A healthcare provider can offer tailored advice and treatment options based on your specific situation.

In conclusion, while some degree of urinary leakage after your first sexual experience can occur, it is essential to monitor your symptoms and seek professional advice if they continue or worsen. Remember, you are not alone in this experience, and many individuals face similar challenges. Taking proactive steps can lead to improvement and peace of mind.
