Do Traditional Chinese Herbs Affect Urination and Cause Swelling? - Traditional Chinese Medicine

Does "Yizhiren," "Wuyao," and "Shanyao" reduce urine output or cause urinary retention? Can they cause peripheral edema?

Grandfather has diabetes and heart disease (he had a mild stroke a month ago).
A few days ago, he visited a traditional Chinese medicine (TCM) practitioner who asked the family if he urinates more at night.
The family responded that he does (though they are not entirely sure, as he might drink a lot in the afternoon and evening, causing him to wake up at night to urinate).
The TCM practitioner prescribed Rou Cong Rong, Bu Yang Huo Wu Decoction, Ejiao, Yi Zhi Ren, Wu Yao, Shan Yao, and Shi Chang Pu.
After taking these medications, grandfather felt some mild swelling in his feet (unsure if it is due to taking Yi Zhi Ren, Wu Yao, and Shan Yao, or because he sat all day without moving).

I have the following questions for the doctor:
1.
Do the prescribed medications cause urinary retention or reduce urination, leading to edema?
2.
I read online that Yi Zhi Ren, Wu Yao, and Shan Yao can cause urinary retention.
What does urinary retention mean? What is its effect? Does it mean that urine is produced but not expelled? Is this good for the body?
3.
I am very concerned that the intention to help grandfather improve his health through TCM may have inadvertently harmed him.
Should I ask the doctor not to prescribe Yi Zhi Ren, Wu Yao, and Shan Yao during the next visit?

Dr. Xu Weihong reply Traditional Chinese Medicine

There are many factors that can cause edema.
Regarding the traditional Chinese medicine you mentioned, there is no documented evidence linking it to edema.
It is recommended that you discuss this with your physician during your next visit to investigate the cause of the edema.

When considering the effects of Traditional Chinese Medicine (TCM) on urination and potential swelling, it is essential to understand both the specific herbs involved and the underlying health conditions of the patient. In your grandfather's case, he has diabetes and a history of heart disease, which can complicate his urinary function and fluid balance.

1. Effects of the Prescribed Herbs: The herbs prescribed to your grandfather—Rou Cong Rong (肉蓯蓉), Bu Yang Huo Wu Tang (補陽還五湯), Ejiao (阿膠), Yi Zhi Ren (益智仁), Wu Yao (烏藥), Shan Yao (山藥), and Shi Chang Pu (石菖蒲)—are traditionally used for various purposes, including tonifying the kidneys, promoting blood circulation, and enhancing overall vitality. However, some of these herbs, particularly Yi Zhi Ren and Wu Yao, are known for their potential effects on urinary function. Yi Zhi Ren is often used to help with urinary incontinence and may have astringent properties that can affect urine output. Wu Yao is known to relieve pain and promote the flow of Qi, but it can also have a mild diuretic effect.

If your grandfather is experiencing swelling in his legs, it could be due to several factors, including prolonged sitting, which can lead to fluid retention, or the effects of the herbs. While these herbs are not typically associated with causing reduced urination directly, they may influence fluid balance in the body, especially in someone with pre-existing conditions like diabetes and heart disease.

2. Understanding "Shrinkage of Urine": The term "shrinkage of urine" (縮尿) refers to a decrease in urine output. This can occur due to various reasons, including dehydration, certain medications, or the effects of specific herbs. In TCM, some herbs may be used to help retain fluids in the body, which can lead to a feeling of reduced urination. However, this is not necessarily beneficial for everyone, especially for someone with a history of heart disease, as it can exacerbate fluid retention and lead to swelling.

3. Concerns About Herbal Treatment: It is understandable to be concerned about the effects of herbal treatments, especially when trying to help a loved one. If you suspect that the herbs are causing adverse effects, such as increased swelling or discomfort, it is crucial to communicate this with the TCM practitioner. They can reassess the treatment plan and make necessary adjustments. It is also important to consider the overall health of your grandfather, including his fluid intake and activity level, as these factors can significantly influence his urinary function and swelling.

In conclusion, while TCM can offer benefits, it is essential to monitor its effects closely, especially in patients with complex health issues. If there are concerns about specific herbs or their effects, do not hesitate to discuss these with the healthcare provider. They can provide guidance on whether to continue or modify the treatment plan, ensuring that it aligns with your grandfather's health needs and safety.
