Can you get pregnant from having sex during menstruation?

Hello Doctor: I would like to ask you a question.
My girlfriend has a very regular menstrual cycle, with a variation of no more than two days.
To avoid the concern of pregnancy, we only have intercourse a day or two before her period and on the fourth or fifth day of her period when the flow is lighter.
We always use non-Okamoto ultra-thin condoms and pay close attention to ensure that the condom does not slip off during intercourse.
We also wash our hands with a cleansing solution before and after intercourse.
Is there a chance of pregnancy with this approach? I often worry about it and it affects my sleep.

Ying, 30~39 year old female. Ask Date: 2021/01/21

Dr. Huang Zifei reply Obstetrics and Gynecology

Dear Ying,
Calculating the safe period for sexual activity without contraception is not a reliable method.
Using condoms throughout intercourse is an effective contraceptive measure.
If you are still feeling anxious and unable to sleep well, I recommend dual contraception—using condoms in combination with either preemptive contraceptive pills or an intrauterine device (IUD).
If you have any further questions, I suggest accompanying your girlfriend to a gynecologist for consultation.

When it comes to the question of whether you can get pregnant during your period, it's important to understand the menstrual cycle and the factors that influence fertility. The menstrual cycle typically lasts about 28 days, but it can vary from person to person. Ovulation, which is when an egg is released from the ovary, usually occurs around the midpoint of the cycle, approximately 14 days before the next period starts. However, sperm can live inside the female reproductive tract for up to five days, which means that if you have unprotected sex just before ovulation, there is a possibility of becoming pregnant.

In your case, since your girlfriend has a very regular menstrual cycle with minimal variation, the likelihood of getting pregnant during her period is relatively low. However, it is not impossible. If you engage in sexual intercourse just before her period ends, and she has a shorter cycle, there is a chance that sperm could still be viable when she ovulates. Additionally, some women may experience irregularities in their cycles, which can make predicting ovulation more challenging.

Using condoms, as you mentioned, is an effective method of contraception when used correctly. Non-latex condoms, like the ones you referred to, can provide a good barrier against sperm, significantly reducing the risk of pregnancy. It's crucial to ensure that the condom is not damaged during use and that it is put on properly before any genital contact occurs. Checking for any signs of breakage or slippage during intercourse is also a good practice.

Regarding your concerns about hygiene, washing your hands before and after sexual activity is a good habit, as it helps reduce the risk of infections. However, it’s essential to note that pregnancy can occur even with careful precautions if there is any failure in the contraceptive method used.

If you and your girlfriend are particularly anxious about the possibility of pregnancy, you might consider additional contraceptive methods. Options such as hormonal birth control pills, intrauterine devices (IUDs), or implants can provide more reliable protection against unintended pregnancy. Consulting with a healthcare provider can help you both understand the best options based on your lifestyle and health needs.

In summary, while the chances of getting pregnant during menstruation are low, they are not zero. Using condoms correctly significantly reduces the risk of pregnancy, and maintaining open communication with your partner about contraception and sexual health is essential. If you continue to have concerns, consider discussing them with a healthcare professional who can provide personalized advice and reassurance.
