Post-operative issues after circumcision?

Hello doctor, I recently underwent a circumcision surgery a few days ago.
Due to severe swelling, it has been very inconvenient to bandage and use the restroom.
Urine often drips onto the gauze, causing me to change the gauze frequently, which is both painful and troublesome.
I would like to ask if the likelihood of urine causing an infection at the wound site is high, or if it's sufficient to just change the gauze occasionally when it only slightly gets wet.

Qing Shao Nian, 20~29 year old female. Ask Date: 2021/01/21

Dr. Li Xinyi reply Urology

If the swelling is severe, it is recommended to position the penis upwards and secure it with 3M tape.
Before changing the dressing, apply ointment to the wound, then cover it with gauze; this will help reduce pain during dressing changes.
Generally, the gauze should be changed when it becomes wet, but since it has been several days post-surgery, the wound should be stable by now.

After undergoing a circumcision, it is common to experience some swelling and discomfort in the surgical area. Your concerns about the potential for infection due to urine exposure are valid, and it's important to address them properly to ensure a smooth recovery.

Firstly, it’s essential to understand that while urine itself is generally sterile when it leaves the body, it can still carry bacteria, especially if there are any underlying infections in the urinary tract. If urine comes into contact with the surgical site, it can potentially introduce bacteria into the wound, increasing the risk of infection. However, the risk of infection from minor exposure, such as urine droplets on the dressing, is relatively low, especially if the dressing is changed promptly and proper hygiene is maintained.

Here are some key points to consider regarding your situation:
1. Swelling and Discomfort: Swelling is a normal part of the healing process after circumcision. It can cause discomfort and may make it challenging to manage the dressing. If the swelling is severe or accompanied by significant pain, it’s advisable to consult your surgeon or healthcare provider.

2. Dressing Changes: If urine frequently drips onto the dressing, it is wise to change it to maintain cleanliness and reduce the risk of infection. While minor exposure may not pose a significant risk, keeping the area clean and dry is crucial for healing. If the dressing becomes wet or soiled, changing it is a good practice.

3. Signs of Infection: Monitor the surgical site for any signs of infection, which may include increased redness, swelling, warmth, discharge (especially if it is yellow or green), or a fever. If you notice any of these symptoms, contact your healthcare provider immediately.

4. Hygiene Practices: Maintain good hygiene by washing your hands before and after touching the surgical area. When changing the dressing, ensure that you are using clean materials and that the area is as sterile as possible. Avoid touching the wound with unwashed hands.

5. Follow-Up Care: Follow any post-operative care instructions provided by your surgeon. This may include recommendations for bathing, dressing changes, and signs to watch for that may indicate complications.

6. Pain Management: If you are experiencing significant pain, discuss pain management options with your healthcare provider. They may recommend over-the-counter pain relievers or prescribe medication to help manage discomfort.

In summary, while the risk of infection from minor urine exposure is low, it is best to err on the side of caution. Regularly changing the dressing when it becomes wet or soiled, maintaining good hygiene, and monitoring for signs of infection will help ensure a smooth recovery. If you have any concerns or if the swelling or discomfort worsens, do not hesitate to reach out to your healthcare provider for further guidance.
