the Side Effects of Shuran Tan in Eye Care - Ophthalmology

The issue with Shuratan?

Dear Director He,
I have researched the side effects of brimonidine (舒而坦) and found that they include deepening of iris and eyelid pigmentation, lengthening of eyelashes, darkening of the skin around the eyes, macular edema, and iritis.
Recent studies have shown that some individuals may experience fat loss around the orbits, leading to upper and lower eyelid ptosis, which can affect vision.
I would like to ask the doctor:
1.
Can brimonidine cause eyelid ptosis?
2.
Will the deepening of iris color return to normal? If not, what are the potential effects?
3.
Will the darkening of eyelid color return to normal? If not, what are the potential effects?
4.
Does apraclonidine (舒露瞳) have fewer side effects? During my last visit, the doctor provided me with two options for my glaucoma: 1.
Apraclonidine 2.
Brimonidine.
Which one should I choose?
Thank you for taking the time to respond amidst your busy schedule.
I am very grateful.

Dr. He Jianxian reply Ophthalmology

Prolonged use of Shuratane can indeed lead to eyelid drooping and darkening of the iris and eyelid color, resembling a smoky makeup effect.
Other effects are minimal.
In contrast, Shulutong does not have these side effects, although it may cause more irritation.

When considering the side effects of Shuran Tan (also known as latanoprost), it is essential to understand its implications for eye care, particularly in patients with glaucoma. Shuran Tan is a prostaglandin analog commonly used to lower intraocular pressure (IOP) in glaucoma patients. However, like any medication, it comes with a range of potential side effects that can impact the patient's quality of life and ocular health.

1. Does Shuran Tan cause eyelid drooping?
Yes, prolonged use of Shuran Tan can lead to eyelid drooping (ptosis) in some individuals. This is often due to the medication's effects on the surrounding tissues and fat around the eyes. The loss of fat in the orbital area can contribute to a more pronounced drooping of the upper and lower eyelids, which may affect vision and the overall appearance of the eyes.

2. Will the deepening of iris color return to normal?
The deepening of iris color, a common side effect of Shuran Tan, is typically irreversible. This change occurs due to increased melanin production in the iris. While this change is generally harmless, it can lead to cosmetic concerns for some patients. The primary impact is aesthetic, but it is essential to monitor any changes in vision or discomfort that may arise.

3. Will the darkening of eyelid color return to normal?
Similar to iris color changes, the darkening of the eyelids can also be permanent. This side effect is often due to increased pigmentation and can be distressing for some patients. While it does not usually have functional implications, it can affect self-esteem and how individuals perceive their appearance.

4. Does Shulu Tong have fewer side effects?
Shulu Tong (also known as brimonidine) is another medication used to manage glaucoma. It generally has a different side effect profile compared to Shuran Tan. While it may cause less pigmentation-related side effects, it can lead to other issues such as ocular irritation or allergic reactions. The choice between these medications should be based on individual patient needs, tolerance, and the specific side effects experienced.

When choosing between Shuran Tan and Shulu Tong for glaucoma management, it is crucial to consider the patient's overall health, the severity of their condition, and their response to previous treatments. Consulting with an ophthalmologist is essential to tailor the treatment plan to the individual’s needs.

In summary, while Shuran Tan is effective in managing intraocular pressure, its side effects, including eyelid drooping and changes in pigmentation, should be carefully considered. Patients should have open discussions with their healthcare providers about their concerns and preferences to ensure the best possible outcomes in their eye care regimen. Regular follow-ups and monitoring are vital to address any emerging issues promptly.
