Issues with emergency contraceptive pills (missed doses and catch-up doses)
Hello, doctor.
I have been taking the oral contraceptive pill "Yueyi" (this is my first time using it, and I started on the fifth day of my menstrual cycle).
I have just completed 7 days of usage, but last week I forgot to bring the pills with me on a business trip, resulting in missing 3 days of doses.
Today, on the fourth day after missing the pills, I have started experiencing light vaginal bleeding.
Can I continue taking the pills now? Is there still a chance to achieve contraceptive effectiveness? Or do I need to wait until my next menstrual cycle to start a new pack? Thank you for your assistance.
Viyo, 20~29 year old female. Ask Date: 2021/02/01

Hello, please do not take it anymore.
Start taking the contraceptive pills again when your next menstrual period arrives.
Wishing you good health.

When it comes to missed doses of oral contraceptives, it's essential to understand the implications for both contraceptive effectiveness and your menstrual cycle. In your case, having missed three consecutive doses of the "Yaz" (or "Yasmin") birth control pill, there are several important considerations to keep in mind.
First, if you miss one or more doses of your birth control pill, the general recommendation is to take the missed pill as soon as you remember. If you miss two or more pills, especially in the first or second week of the pack, you should take the most recent missed pill as soon as you remember and discard any earlier missed pills. After that, continue taking the remaining pills at your regular schedule. However, if you miss pills in the third week, it's often advised to skip the placebo pills and start a new pack immediately to maintain contraceptive protection.
Given that you missed three pills, your contraceptive effectiveness may be compromised. The risk of unintended pregnancy increases significantly with missed doses, particularly if you had unprotected intercourse during the time you missed your pills. Since you have started experiencing light vaginal bleeding, this could be a sign of withdrawal bleeding due to the hormonal fluctuations caused by missing the pills. It’s not uncommon for women to experience irregular bleeding when they miss doses, especially if they are new to hormonal contraceptives.
Now, regarding your question about whether you can continue taking the pills: Yes, you can resume taking your pills. However, since you have missed three doses, you should also consider using a backup method of contraception, such as condoms, for at least the next seven days to ensure you are protected against pregnancy. If you have had unprotected intercourse during the time you missed your pills, you may want to consider emergency contraception, especially if you are within the appropriate time frame (usually within 72 to 120 hours after unprotected sex).
As for starting a new pack, it is generally recommended to finish the current pack you have, even if it means taking the remaining pills after the bleeding occurs. If you find that your bleeding continues or becomes heavy, or if you have any other concerning symptoms, it would be wise to consult with your healthcare provider. They can provide personalized advice and may suggest alternative contraceptive methods if necessary.
In summary, here are the steps you should follow:
1. Take the most recent missed pill as soon as you remember and continue taking the remaining pills in the pack.
2. Use a backup contraceptive method for at least seven days.
3. If you had unprotected intercourse during the missed doses, consider emergency contraception.
4. Monitor your bleeding and any other symptoms, and consult your healthcare provider if you have concerns.
It's crucial to maintain a consistent routine with your birth control pills to maximize their effectiveness. If you find that you frequently forget to take your pills, you might want to discuss alternative contraceptive methods with your healthcare provider, such as the patch, ring, or IUD, which may be more suitable for your lifestyle.
