Brown discharge a few days before menstruation?

Hello, doctor.
I usually take birth control pills (Yaz), and my period is expected to start this Saturday.
However, since last Saturday, I have noticed dark brown discharge (very minimal amount).
My last two sexual encounters were between January 13-15 and January 23-25.
On the 23rd, I missed a pill but took it the next day within 24 hours (including the one I was supposed to take that day, for a total of two pills).
What could this discharge indicate? Additionally, would you recommend getting checked before my period or waiting until after my period to observe? Thank you!

M, 20~29 year old female. Ask Date: 2021/02/04

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, if you haven't had this issue while taking birth control pills before, it's fine to observe for now.
However, if this occurs frequently, I recommend trying a different type of birth control pill.
Wishing you good health.

Brown discharge before menstruation can be a common occurrence and is often not a cause for concern, especially for individuals who are on hormonal contraceptives like the pill you mentioned (Yasmin). The brown color typically indicates older blood that has taken longer to exit the uterus, which can happen for various reasons. Here’s a breakdown of potential causes and advice regarding your situation.

Causes of Brown Discharge
1. Hormonal Fluctuations: Since you are on birth control pills, your body is undergoing hormonal changes that can lead to spotting or brown discharge. This is particularly common during the first few months of starting a new contraceptive method or if there are missed doses, as you experienced.

2. Implantation Bleeding: If there is a possibility of pregnancy, brown discharge could be a sign of implantation bleeding, which occurs when a fertilized egg attaches to the lining of the uterus. However, given that you are on birth control, this is less likely unless there was a significant lapse in your contraceptive method.

3. Menstrual Cycle Changes: As your menstrual cycle progresses, the endometrial lining sheds, and older blood may take longer to exit the body, resulting in brown discharge just before your period starts.

4. Infections or Other Conditions: While less common, brown discharge can also be a sign of infections or other medical conditions such as endometriosis or fibroids. If accompanied by other symptoms like unusual odor, itching, or pain, it would warrant further investigation.

Advice
- Monitor Your Symptoms: Since you mentioned that the discharge is minimal and you are expecting your period soon, it may simply be a part of your menstrual cycle. Keep track of the discharge and any other symptoms you may experience.

- Continue Taking Your Pills: Ensure that you continue taking your birth control pills as prescribed. Missing doses can lead to irregular bleeding or spotting.

- Consult Your Healthcare Provider: If the brown discharge persists beyond your expected period, increases in volume, or is accompanied by other concerning symptoms (such as pain or unusual odor), it would be wise to consult your healthcare provider. They may recommend a pelvic exam or other tests to rule out any underlying issues.

- Timing of Check-ups: If your symptoms do not resolve after your period, or if you have any concerns about your menstrual cycle or contraceptive method, consider scheduling an appointment with your healthcare provider. It’s often beneficial to wait until after your period to assess any changes, but don’t hesitate to seek advice sooner if you feel it’s necessary.

Conclusion
In summary, brown discharge before menstruation can be a normal part of your menstrual cycle, especially when on hormonal contraceptives. However, it’s important to remain vigilant about your health and consult with a healthcare provider if you have any concerns or if symptoms change. Regular check-ups and open communication with your doctor can help ensure that your reproductive health is well managed.
