Lower abdominal tenderness with pain on movement?

Hello doctor, I am currently 16 weeks pregnant and have suddenly experienced pain in my lower abdomen.
Pressing on my lower abdomen, turning over, walking, sneezing, and straining during bowel movements all cause significant pain.
I visited an obstetrician the next day, and the doctor indicated that the pelvic ultrasound looked fine and suggested that it might be ligament stretching.
They prescribed medication to support the pregnancy and advised me to rest more.
However, today marks the fourth day of pain, and there has been no improvement; I also have difficulty sleeping at night due to the pain.
Recently, I noticed that shortly after eating, I need to rush to the bathroom, and my stool is chocolate-colored.
I am not experiencing diarrhea, but the latter part of the stool feels stickier, and my appetite is normal.
The most painful area upon palpation is the center of my lower abdomen, with tenderness also present on both sides.
Is this a gastrointestinal issue? Should I see a gastroenterologist?

It doesn't seem like a gastrointestinal issue.
If possible, a stool occult blood test can be performed to rule out this possibility.

Experiencing lower abdominal pain during pregnancy can be concerning, especially when it persists for several days. At 16 weeks pregnant, it is essential to understand the potential causes of your pain and when to seek further medical help.

Firstly, it is not uncommon for pregnant women to experience abdominal pain due to various reasons, including ligament stretching, which occurs as the uterus expands. This type of pain is typically sharp and localized, often described as a pulling sensation. However, if the pain is severe, persistent, or accompanied by other symptoms, it may indicate a more serious issue.

In your case, since you have already consulted with a healthcare provider who performed an ultrasound and found no immediate concerns, it is likely that the pain is related to the normal changes of pregnancy. However, the fact that the pain has not improved after several days and is affecting your ability to sleep warrants further investigation.
The symptoms you describe, such as the pain worsening with movement, pressure, or certain activities, could still be attributed to ligament stretching. However, the presence of chocolate-colored stools and changes in bowel habits could suggest gastrointestinal issues. While chocolate-colored stools can sometimes indicate bleeding in the upper gastrointestinal tract, it is crucial to consider other factors, such as dietary changes or the effects of prenatal vitamins, which can sometimes alter stool color.

Given your symptoms, it may be prudent to consult a gastroenterologist if the abdominal pain persists or worsens. They can evaluate whether there is an underlying gastrointestinal issue that needs to be addressed. Additionally, if you experience any new symptoms such as fever, vomiting, or significant changes in bowel habits, you should seek immediate medical attention.

In terms of managing your abdominal pain, resting and avoiding activities that exacerbate the pain is advisable. Heat therapy, such as a warm compress on the abdomen, may provide some relief. However, be cautious with any medications, including over-the-counter pain relievers, as not all are safe during pregnancy. Always consult your healthcare provider before taking any medication.

It is also essential to monitor your blood pressure and overall health during this time. Your previous reading of 97/59 is on the lower side, which could contribute to feelings of fatigue or dizziness. Staying hydrated and eating small, frequent meals can help maintain your energy levels and manage any gastrointestinal discomfort.

In summary, while lower abdominal pain during pregnancy can often be attributed to normal physiological changes, persistent or severe pain should not be ignored. If your symptoms do not improve or if you have concerns about your gastrointestinal health, seeking further evaluation from a specialist is advisable. Always prioritize open communication with your healthcare provider to ensure both your health and the health of your baby are monitored closely.
