Drug interactions with oral isotretinoin?

Hello Doctor, I have been reading many articles but couldn't find information regarding the interaction between isotretinoin (A acid) and oral contraceptives, so I am reaching out to you.
My skin has not been good lately, and I have been experiencing acne, so I visited a dermatologist who prescribed doxycycline and oral isotretinoin.
I have been taking Yasmin for a month now and am currently on the sixth pill of the second pack.
I have two questions for you:
1.
I had intercourse with my boyfriend on the first day of my second pack (fourth day of my menstrual period) while he was wearing a condom, but he mentioned that it slipped off during the act.
Could this lead to pregnancy? If I trust the efficacy of the contraceptive pill, could the antibiotics I am taking from the dermatologist affect its effectiveness? I take the antibiotics after dinner and the contraceptive pill before bed, with at least a 3-4 hour gap in between.
2.
I just visited a new dermatologist today, and he prescribed doxycycline.
Will this conflict with my contraceptive pill? Should I wait a few hours after taking doxycycline before taking my contraceptive pill to avoid any interaction? Thank you, Doctor!

Hello, taking birth control pills on time will provide effective contraception.
The medication you are taking generally will not affect the contraceptive efficacy.
Wishing you good health.

More Info

When it comes to understanding drug interactions, particularly between oral retinoids (like isotretinoin) and birth control pills, it's essential to consider how these medications work and their potential effects on each other.

1. Effectiveness of Birth Control Pills: Birth control pills, such as the one you mentioned (美適農), are designed to prevent ovulation and thus prevent pregnancy. When taken consistently and correctly, they are highly effective. However, certain medications can interfere with their effectiveness.
2. Antibiotics and Birth Control: There is a common misconception that all antibiotics reduce the effectiveness of birth control pills. While some antibiotics, like rifampin, can indeed lower the effectiveness of hormonal contraceptives, most commonly prescribed antibiotics, including doxycycline, do not have this effect. Therefore, if you are taking doxycycline for acne, it should not affect the efficacy of your birth control pill.
3. Timing of Medication: You mentioned that you take your birth control pill at night and doxycycline after dinner. This timing is generally acceptable and should not cause any issues. The key is to maintain a consistent schedule for both medications. As long as you are taking them as prescribed and there is a gap of several hours between them, you should be fine.

4. Contraceptive Failure Concerns: Regarding your concern about the condom slipping during intercourse, it is important to note that if you have been taking your birth control pill consistently for at least seven days prior to having unprotected intercourse, you should still be protected against pregnancy. Since you mentioned that you were on your second pack of birth control and had been taking it for a month, the likelihood of pregnancy is low, even with the condom slipping. However, if you are still concerned, you might consider taking a pregnancy test if your next period is late.

5. Consulting Your Healthcare Provider: It’s always a good idea to discuss any new medications with your healthcare provider, especially when you are on multiple prescriptions. They can provide personalized advice based on your medical history and current medications. If you have any unusual symptoms or concerns while taking these medications, do not hesitate to reach out to them.

6. Additional Considerations: While doxycycline is generally safe to take with birth control pills, it’s important to avoid certain interactions with other medications or supplements. For example, taking certain supplements like calcium or magnesium close to the time you take doxycycline can reduce its absorption. Always check with your healthcare provider or pharmacist about potential interactions with any new medications or supplements you might consider.

In summary, based on current medical knowledge, taking doxycycline alongside your birth control pill should not compromise its effectiveness. As long as you maintain a consistent schedule and communicate with your healthcare provider about any concerns, you should be well-protected against unintended pregnancy. If you have further questions or experience any side effects, please consult your healthcare provider for tailored advice.
