Prolonged menstrual period

Hello, doctor.
During my health check last May, a cyst was found on my ovary.
After three months, I visited a gynecologist, and it was still there.
I have been undergoing blood tests, and all tumor markers were normal.
Later, I went to a Chinese medicine clinic, where the doctor said it was a benign tumor.
I have been continuously monitored, and the doctor advised follow-ups every 3 to 6 months.
My last check-up at the Chinese medicine clinic was on December 15, where a transvaginal ultrasound showed the cyst was still 4.9 cm.
My usual menstrual cycle is 33 days, lasting 7 days, but this month my period was 5 days late.
The first 4 days had heavier flow, but now on the 9th day, it hasn't completely cleared up, and I change my pad about 1 to 2 times a day, with not much flow.
Occasionally, I feel lower back pain.
Should I see a doctor now, or should I continue to observe?

S, 20~29 year old female. Ask Date: 2021/02/19

When it comes to prolonged menstrual periods, it is essential to understand the potential implications and when to seek medical advice. Based on your description, you have a history of ovarian cysts that have been monitored and deemed benign. However, your current situation of a delayed menstrual cycle, heavy bleeding for the first four days, and ongoing bleeding for nine days warrants careful consideration.

Firstly, a menstrual period that lasts longer than seven days is generally considered prolonged. In your case, since you are already on day nine of bleeding, it is advisable to seek medical attention. While some variations in menstrual cycles can occur due to stress, hormonal changes, or other benign factors, prolonged bleeding can sometimes indicate underlying issues that require further evaluation.

Here are some reasons why you should consider seeing a healthcare provider:
1. Assessment of Bleeding: Prolonged bleeding can lead to anemia, especially if the flow is heavy. Symptoms of anemia include fatigue, weakness, and dizziness. If you notice any of these symptoms, it is crucial to seek medical help promptly.

2. Monitoring of Ovarian Cysts: Although your previous evaluations indicated that your ovarian cysts are benign, any changes in your menstrual cycle could be related to these cysts. It is essential to have a healthcare provider assess whether the cysts have changed in size or nature, which could be contributing to your symptoms.

3. Hormonal Imbalances: Prolonged menstrual bleeding can sometimes be a sign of hormonal imbalances, such as those caused by conditions like polycystic ovary syndrome (PCOS) or thyroid disorders. A healthcare provider can perform blood tests to evaluate hormone levels and determine if further treatment is necessary.

4. Other Potential Causes: There are various other conditions that can cause prolonged menstrual bleeding, including fibroids, endometriosis, or infections. A thorough examination and possibly imaging studies can help rule out these conditions.

5. Pain Management: You mentioned experiencing lower back pain, which can accompany prolonged menstrual bleeding. A healthcare provider can offer pain management strategies and assess whether the pain is related to your menstrual cycle or another underlying issue.

In summary, given your history of ovarian cysts, the current prolonged menstrual bleeding, and associated symptoms, it is advisable to consult with your healthcare provider. They can perform a comprehensive evaluation, including a physical examination and possibly imaging or blood tests, to determine the cause of your symptoms and recommend appropriate treatment. Early intervention can help prevent complications and provide peace of mind regarding your reproductive health.
Remember, it is always better to err on the side of caution when it comes to your health, especially with changes in your menstrual cycle. Seeking medical advice will ensure that you receive the appropriate care and guidance tailored to your specific situation.
