Impact to the back of the head?
Hello Doctor,
I have a question I would like to ask you.
While playing basketball, I accidentally fell backward and hit the back of my head hard on the floor.
I immediately felt very uncomfortable and dizzy, so I lay down to rest.
However, when I woke up, my right hand was numb.
After resting for a day, the dizziness improved significantly the next day, but on the third day, I still felt a bit dizzy.
Additionally, the area where I hit my head now hurts even when I lie down at night, and I can feel pain when I touch it.
Is this what is referred to as a concussion? Is it a problem that I haven't had a CT scan? Do I need to get a CT scan? Would it be useful to get one if I didn't go on the day of the injury? If I need to get a CT scan, which department should I visit?
Thank you very much for helping me clarify my concerns.
I am infinitely grateful.
Thank you.

Hello: In response to your question about whether a CT scan is necessary for head trauma, several factors need to be considered, including the time elapsed since the injury, whether there was a loss of consciousness, any neurological deficits, and related symptoms.
If symptoms persist, please visit a neurosurgery outpatient clinic for further evaluation.

More Info
Concussions are a type of traumatic brain injury (TBI) that can occur after a blow to the head or body, causing the brain to move rapidly within the skull. Symptoms can vary widely and may include headache, dizziness, confusion, nausea, balance problems, and even numbness or tingling in the extremities, as you have experienced with your right hand. It’s important to understand the signs of a concussion and when to seek medical attention, including imaging studies like CT scans.
Based on your description, it sounds like you may have sustained a concussion. The immediate symptoms you experienced—dizziness and discomfort after hitting your head—are common indicators. The subsequent numbness in your hand and persistent pain at the site of impact are concerning and warrant further evaluation. While some concussion symptoms may improve within a day or two, others can persist or even worsen, indicating that a more serious injury may have occurred.
In general, a CT scan is recommended in the following situations after a head injury:
1. Loss of Consciousness: If you were knocked out or experienced a significant loss of consciousness.
2. Severe Headache: If you develop a severe headache that does not improve with over-the-counter pain relief.
3. Neurological Symptoms: If you experience symptoms such as persistent dizziness, confusion, difficulty speaking, weakness, or numbness in any part of your body.
4. Worsening Symptoms: If your symptoms worsen over time rather than improve.
Given that you are still experiencing dizziness and pain at the site of impact, it would be prudent to seek medical attention. A CT scan can help rule out any serious injuries, such as bleeding in the brain or skull fractures. It’s important to note that while a CT scan is most effective when performed soon after the injury, it can still provide valuable information even a few days later if symptoms persist.
You should visit a healthcare provider who specializes in head injuries, such as a neurologist or an emergency medicine physician. They can assess your symptoms, perform a neurological examination, and determine whether a CT scan is necessary. If you are experiencing significant discomfort or neurological symptoms, it may be best to go to the emergency room for immediate evaluation.
In conclusion, while some concussion symptoms may resolve quickly, persistent symptoms like yours should not be ignored. Seeking medical evaluation and possibly a CT scan is a wise decision to ensure your safety and health. Remember, it’s always better to err on the side of caution when it comes to head injuries.
