Arc-Shaped Flashes After Retinal Laser Surgery - Ophthalmology

After retinal laser surgery, there may be arc-shaped flashes of light?

Hello, Doctor Gao.
I would like to consult with you: Recently, while walking, I occasionally noticed arc-shaped flashes in the upper part of my right eye.
After an examination at the hospital, it was found that there was retinal degeneration and a tear.
That same day, I underwent laser surgery to seal the tear.
However, a few days later, the arc-shaped flashes reappeared.
Two weeks later, during a follow-up visit, the scabbing status was not as expected, so I received a second round of laser treatment.
Within a week after the second treatment, the original flashes transformed into black shadows.
During the follow-up, the doctor mentioned that there was no need for concern and that I should return for another check-up in six months.
However, a week later, the frequency of the arc-shaped flashes in the upper part of my right eye increased.
I would like to ask the doctor if it is normal to experience such flashes after laser treatment, or if this indicates another issue unrelated to the laser surgery? Will this condition improve over time?

Hello: The laser is applied around the retinal tear, and the flashes are caused by the vitreous pulling on the retina.
Therefore, even after the laser treatment for the tear, if the pulling continues, flashes may still occur.
However, because the laser treatment causes scarring on the retina, it becomes less likely for the pulling to lead to retinal detachment.
This condition may sometimes improve, but it can also persist.

After undergoing retinal laser surgery, experiencing arc-shaped flashes can be concerning, especially when they persist or increase in frequency. It is essential to understand the underlying mechanisms and potential implications of these symptoms.

Firstly, the appearance of arc-shaped flashes, particularly after retinal surgery, may be attributed to several factors. Retinal detachment or degeneration can lead to the formation of tears or holes in the retina, which can cause visual disturbances such as flashes of light. These flashes occur due to the stimulation of the retina's photoreceptors, often triggered by movement or changes in the vitreous gel that fills the eye. When the vitreous pulls away from the retina, it can create sensations of light, often described as flashes or streaks.

In your case, the initial laser treatment aimed to seal the retinal tears and prevent further detachment. However, the recurrence of arc-shaped flashes suggests that there may still be some ongoing vitreous traction or changes in the retinal structure. The fact that the flashes have transformed into black shadows after the second laser treatment indicates that there may be some residual effects from the surgery or changes in the vitreous gel.

It is important to note that while some degree of visual disturbance can be expected after retinal surgery, persistent or worsening symptoms should be closely monitored. The increase in frequency of the arc-shaped flashes you are experiencing could be related to several factors, including:
1. Vitreous Changes: After surgery, the vitreous gel may continue to change, leading to further traction on the retina. This can result in new flashes or changes in existing ones.

2. Healing Process: The retina and surrounding tissues undergo a healing process after laser treatment. During this time, it is not uncommon for patients to experience fluctuations in their symptoms as the tissues adjust.

3. Potential Complications: Although the laser treatment aims to prevent complications, there is still a risk of new tears or detachment occurring. If the flashes are accompanied by other symptoms such as a sudden increase in floaters, a shadow in your vision, or a curtain-like effect, it is crucial to seek immediate medical attention.

Regarding your question about whether these flashes are normal after laser surgery, it is essential to communicate closely with your ophthalmologist. They can assess the status of your retina and determine if the flashes are a normal part of the healing process or if they indicate a need for further intervention.

In terms of improvement, many patients find that their symptoms gradually decrease as the eye heals and the vitreous stabilizes. However, this can vary significantly from person to person. Regular follow-up appointments are vital to monitor your condition and ensure that any potential issues are addressed promptly.

In summary, while experiencing arc-shaped flashes after retinal laser surgery can be a normal part of the healing process, persistent or worsening symptoms warrant further evaluation. It is crucial to maintain open communication with your eye care provider, who can offer tailored advice and treatment options based on your specific situation. Regular monitoring and follow-up care are essential to ensure the best possible outcome for your vision.
