Ear and throat pain?

Hello Doctor, I have been experiencing tinnitus since before the Lunar New Year.
I have a persistent dull pain in the tonsils at the back of my tongue, and there is also pain in the area indicated by the arrow in the photo.
Sometimes, I experience sudden ear pain, but recently the ear pain has intensified to the point where I cannot sleep.
The tinnitus has not stopped, and my ears feel swollen with a crackling sound.
Touching my ears with my fingers also causes pain, and there is pain below the ear as well.
I visited an otolaryngologist, who diagnosed me with temporomandibular joint pain and prescribed medication.
After taking the medication for three days, the pain seemed less intense, but I still experience persistent sharp pain, tinnitus, and a feeling of fullness in my ears.
I went back to the otolaryngologist, and this time the doctor suggested it might be a nerve issue and prescribed another three days of medication.
I am currently taking the medication and would like to consult with you because I am worried that there might be something wrong in my oral cavity or ear that could be causing the symptoms of tinnitus, ear pain, and fullness, which are not visible externally.
The symptoms are ongoing, and it is quite uncomfortable.
I would like to know your suggestions or analysis of my condition and what tests I should undergo.
The doctor previously used a rigid endoscope to examine my throat and said it looked fine, but I still have throat pain along with ear pain, tinnitus, and a feeling of fullness.
I am really concerned about what the problem might be.
All of these symptoms are on the left side of my throat and ear.
I have attached a photo for your review.
Thank you.

Understanding ear and throat pain can be quite complex, as these symptoms can arise from a variety of underlying conditions. Based on your description, it seems you are experiencing a combination of ear pain, tinnitus (ear ringing), and throat discomfort, particularly on the left side. These symptoms can be interrelated and may stem from several potential causes.

Possible Causes
1. Eustachian Tube Dysfunction: The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure. If this tube is not functioning properly, it can lead to a sensation of fullness in the ear, pain, and tinnitus. This dysfunction can be caused by allergies, sinus infections, or upper respiratory infections.

2. Temporomandibular Joint (TMJ) Disorders: You mentioned a diagnosis of TMJ pain. This condition can cause pain in the jaw joint and surrounding areas, including the ears and throat. TMJ disorders can lead to referred pain, where discomfort is felt in areas other than where the problem originates.

3. Tonsillitis or Pharyngitis: Since you have persistent pain in the area of the tonsils, it’s possible that you are experiencing chronic tonsillitis or pharyngitis. Inflammation of these areas can lead to referred pain in the ear, as the nerves in these regions are interconnected.

4. Nerve Issues: Your doctor suggested that there might be a neurological component to your symptoms. Conditions such as neuralgia (nerve pain) can cause sharp, stabbing pain in the ear and throat. This could be due to irritation or inflammation of the nerves that supply these areas.

5. Infections: Both viral and bacterial infections can lead to ear pain and throat discomfort. If you have had a recent upper respiratory infection, it could have led to secondary infections in the ear or throat.

Symptoms Analysis
- Ear Pain: The intensity of your ear pain, especially if it disrupts your sleep, is concerning. The fact that it is accompanied by tinnitus and a sensation of fullness suggests that there may be fluid buildup or pressure changes in the ear.

- Throat Pain: The persistent pain in your throat, particularly around the tonsils, could indicate chronic inflammation or infection. If the pain is sharp and intermittent, it may be related to nerve irritation.

- Tinnitus: The ongoing ringing or buzzing in your ear can be a result of various factors, including exposure to loud noises, earwax buildup, or even stress.

Recommendations for Further Evaluation
1. Audiological Assessment: A hearing test can help determine if there is any underlying hearing loss or other auditory issues contributing to your symptoms.

2. Imaging Studies: If the symptoms persist, consider discussing the possibility of imaging studies, such as an MRI or CT scan, to rule out structural issues or abnormalities in the ear, throat, or surrounding areas.

3. Referral to a Specialist: If your symptoms do not improve with initial treatments, a referral to a neurologist or an ear, nose, and throat (ENT) specialist may be warranted for further evaluation.

4. Allergy Testing: If Eustachian tube dysfunction is suspected, allergy testing may help identify any allergens that could be contributing to your symptoms.

5. Pain Management: Discuss with your doctor about appropriate pain management strategies, which may include medications, physical therapy for TMJ issues, or other interventions.

Conclusion
Your symptoms of ear pain, tinnitus, and throat discomfort are interconnected and may arise from a combination of factors. It is crucial to work closely with your healthcare provider to explore these potential causes and receive appropriate treatment. Keeping a symptom diary, noting when symptoms worsen or improve, can also be helpful in identifying triggers and guiding your healthcare team in managing your condition effectively.
