Menstrual-related issues?

Hello doctor, I usually have light menstrual flow, with my periods lasting about three days each time, and I feel like I don't need to change my sanitary pads frequently.
However, this month, I have some concerns.
On the first day of my period, the flow was heavy and accompanied by many blood clots, with the clots being larger than a five-dollar coin.
However, by the time I went to bed, the bleeding had stopped.
In the past, I occasionally experienced heavier flow with blood clots, but it was never as severe as this time.
I'm unsure if this issue with the blood clots requires a hospital examination, or if my long-term light flow also needs to be checked.

Abnormal menstrual bleeding and the presence of blood clots can be concerning, especially when there is a significant change in your usual menstrual pattern. In your case, you mentioned that your periods are typically light and last about three days, but this month you experienced a heavier flow with large blood clots. This change warrants attention, and it’s important to understand the potential causes and when to seek medical evaluation.

Firstly, menstrual bleeding can vary significantly from month to month due to a variety of factors, including hormonal fluctuations, stress, diet, and underlying health conditions. The presence of large blood clots, particularly those larger than a quarter (or five-cent coin), can indicate that your body is expelling a larger volume of blood than usual, which may be due to several reasons:
1. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or perimenopause can lead to irregular menstrual cycles and changes in flow. Hormonal imbalances can cause the lining of the uterus to build up more than usual, leading to heavier bleeding when it is shed.

2. Uterine Fibroids or Polyps: These benign growths in the uterus can cause heavier menstrual bleeding and the formation of blood clots. They can vary in size and may not always cause symptoms, but when they do, they can significantly affect menstrual flow.

3. Endometriosis: This condition occurs when tissue similar to the lining of the uterus grows outside of it, leading to painful periods and heavy bleeding. It can also cause blood clots.

4. Coagulation Disorders: If you have a family history of bleeding disorders or if you experience other symptoms such as easy bruising or prolonged bleeding from cuts, it may be worth investigating potential clotting disorders.

5. Miscarriage or Ectopic Pregnancy: If there is a possibility of pregnancy, heavy bleeding with clots could indicate a miscarriage or an ectopic pregnancy, which is a medical emergency.

Given your description, it is advisable to seek medical attention. A healthcare provider can perform a thorough evaluation, which may include a physical examination, blood tests to check for anemia or hormonal levels, and possibly imaging studies like an ultrasound to assess the uterus and ovaries.
In terms of your long-term history of light menstrual flow, it is also worth discussing with your doctor. While light periods can be normal for some women, any significant changes in your menstrual pattern, especially if accompanied by other symptoms (such as pain, fatigue, or changes in weight), should be evaluated to rule out any underlying conditions.

In conclusion, while occasional changes in menstrual flow can be normal, the combination of heavy bleeding with large clots is a signal to consult a healthcare professional. Early evaluation can help identify any potential issues and guide appropriate treatment, ensuring your reproductive health is maintained. Remember, your health is important, and it’s always better to err on the side of caution when it comes to changes in your body.
