Medical consultation

Hello Doctor, I apologize for the interruption.
As an engineer, I need to use my eyes for extended periods, and I also have quite severe floaters.
Recently, I have experienced the following situations and would like to consult you:
1.
When looking at white LED lights or white fluorescent lights, I notice a very distinct rainbow around them.
Additionally, I have been feeling somewhat dizzy frequently.
I researched online and found that this might be related to glaucoma.
I went to the hospital for a dilated eye exam, but the doctor only mentioned that my optic nerve is normal.
Should I still go to a larger hospital for a visual field test? Thank you!
2.
Recently, even when my eyes are closed, I still perceive a bright light in front of me, which seems to linger and doesn't go away.
Sometimes, when I wake up in the morning, everything still appears very bright.
Occasionally, when I sleep, I see a blue halo that keeps spreading.
I went to the hospital for a dilated eye exam, but the doctor did not provide a detailed explanation.
I would like to know what could cause this situation.
I am a bit worried that there might be an issue with my photoreceptor layer.
Is there any way to improve this? Thank you!

Hello Engineer: 1.
The appearance of rainbows may also indicate glaucoma; if possible, you should have your visual fields checked.
2.
This could be a form of photopsia, which may not be detectable and is often a physiological phenomenon.

More Info

Your concerns regarding your eye symptoms, particularly floaters, glare, and potential vision issues, are valid and warrant careful consideration. As an engineer who spends long hours using your eyes, it’s essential to understand the implications of these symptoms and how to manage them effectively.

Understanding Your Symptoms
1. Floaters: Floaters are tiny spots, lines, or cobweb-like shapes that drift through your field of vision. They are often caused by changes in the vitreous gel that fills the eye, which can become more liquid and form clumps as we age. In your case, having a history of significant floaters suggests that you may be experiencing normal age-related changes or possibly more serious conditions like vitreous detachment. While floaters are common and usually harmless, a sudden increase in floaters or the appearance of flashes of light should prompt immediate medical attention, as they can indicate retinal issues.

2. Glare and Rainbow Effects: The rainbow halos around lights, especially at night, can be attributed to several factors, including corneal irregularities, dry eyes, or even the effects of high myopia (nearsightedness). Given your history of high myopia, it’s possible that changes in your eye structure are contributing to these visual disturbances. Glare can also be exacerbated by dry eyes, which is a common condition among those who spend extended periods staring at screens.

3. Dizziness and Visual Disturbances: The sensation of dizziness and visual disturbances when looking at bright lights can be concerning. While it’s good that your optic nerve was found to be normal during your examination, persistent dizziness may require further investigation. It could be related to ocular migraines, which can cause temporary visual disturbances and are often accompanied by headaches, or it could be a sign of other neurological issues.

4. Persistent Brightness and Color Halos: The experience of seeing persistent brightness or halos, even with your eyes closed, is unusual and should be evaluated further. This could be related to photophobia (light sensitivity) or other retinal issues. The presence of blue halos may suggest a problem with the cornea or lens, and it’s essential to rule out any serious conditions.

Recommendations
1. Follow-Up with an Eye Specialist: Given your symptoms, it would be prudent to follow up with an ophthalmologist, especially one who specializes in retinal diseases. A comprehensive eye exam, including a visual field test, can help assess your peripheral vision and detect any abnormalities that may not have been evident in previous examinations.

2. Consider Visual Ergonomics: As an engineer, you may be spending long hours in front of screens. Implementing the 20-20-20 rule can help alleviate eye strain: every 20 minutes, look at something 20 feet away for at least 20 seconds. Additionally, ensure that your workspace is well-lit and that you take regular breaks to reduce eye fatigue.

3. Manage Dry Eye Symptoms: If you suspect dry eyes are contributing to your glare and discomfort, consider using artificial tears or lubricating eye drops. Staying hydrated and using a humidifier in your workspace can also help.

4. Monitor Symptoms: Keep a detailed log of your symptoms, including when they occur, their duration, and any associated factors (like screen time or lighting conditions). This information can be invaluable for your healthcare provider in diagnosing and managing your condition.

5. Lifestyle Adjustments: Ensure you are getting adequate sleep, managing stress, and maintaining a balanced diet rich in antioxidants, which can support eye health. Regular exercise can also improve circulation and overall well-being.

Conclusion
While your symptoms may be concerning, many eye conditions can be managed effectively with the right approach. Regular check-ups with an eye care professional, combined with lifestyle adjustments, can significantly improve your eye health and comfort. If your symptoms persist or worsen, do not hesitate to seek further evaluation. Your vision is invaluable, and proactive management is key to preserving it.
