Abnormal discharge

Hello: I previously took oral contraceptives regularly with a 28-day cycle, but I have decided to stop taking them and let my menstruation occur naturally.
My last menstrual period started on February 15.
I had unprotected intercourse with ejaculation on February 20, and there has been no sexual activity since then.
Starting from March 13, I noticed brown discharge, and when I wiped, there was a very small amount of red discharge.
On March 15, I took a pregnancy test, which showed one line.
I would like to know if the discharge I experienced is related to the premenstrual phase or if it could be implantation bleeding.
Since it has been 23 days since intercourse and the test was negative, does this rule out pregnancy?

A. H, 40~49 year old female. Ask Date: 2021/03/16

Dr. Lin Manying reply Obstetrics and Gynecology

Ms.
A.H: Hello, based on your description, here are the responses: 1.
It is normal to experience lighter menstrual periods shortly after stopping birth control pills.
2.
If the pregnancy test on March 15 is negative, and another test three days later is also negative, then the March 15 period is considered light (due to stopping the birth control).
3.
If you take another pregnancy test more than 14 days later and it is still negative, it is confirmed as negative.

Understanding the difference between menstrual bleeding and implantation bleeding can be crucial for individuals trying to determine their reproductive health status. Based on your situation, let’s break down the details.

You mentioned that you have a regular menstrual cycle of 28 days and that you stopped taking your birth control pills. Your last menstrual period (LMP) was on February 15, and you had unprotected intercourse on February 20. Following that, you started noticing brown discharge or very light red spotting beginning on March 13, which is about 26 days after your last period. You took a pregnancy test on March 15, which returned a negative result.

Menstrual Bleeding vs. Implantation Bleeding
1. Menstrual Bleeding: This typically occurs every 28 days for those with regular cycles, lasting anywhere from 3 to 7 days. The flow can vary from light to heavy and is usually accompanied by other symptoms such as cramping, bloating, and mood changes.

2. Implantation Bleeding: This occurs when a fertilized egg attaches to the lining of the uterus, which can happen about 6 to 12 days after conception. Implantation bleeding is generally lighter than a regular period and may appear as light spotting or brown discharge. It usually lasts for a few hours to a couple of days.

Your Situation
Given that you had unprotected intercourse shortly after stopping the pill, there is a possibility of pregnancy. However, the timing of your symptoms is essential. The brown discharge you are experiencing could be a sign of either:
- Pre-menstrual Symptoms: Since your menstrual cycle is regular, the spotting could indicate that your period is approaching. Spotting before a period can occur for various reasons, including hormonal fluctuations after stopping birth control.

- Implantation Bleeding: If you conceived during the unprotected intercourse, the spotting could also be implantation bleeding. However, this would typically occur earlier than the 26-day mark post-LMP.

Pregnancy Test Interpretation
You mentioned that you took a pregnancy test on March 15, which was 23 days after intercourse and 28 days after your last period. A negative pregnancy test at this point is generally reliable, as most home pregnancy tests can detect pregnancy hormones (hCG) about a week after a missed period. Since your test was negative, it suggests that you are likely not pregnant, especially if your cycle is regular.

Recommendations
1. Monitor Your Symptoms: Keep track of the discharge. If it turns into a heavier flow or if you experience any unusual symptoms (like severe cramping or fever), it would be wise to consult a healthcare provider.

2. Wait for Your Period: If your period does not arrive within a week or so, consider taking another pregnancy test for confirmation.
3. Consult a Healthcare Provider: If you have concerns about your menstrual cycle, the effects of stopping birth control, or if you experience significant changes in your discharge, it’s advisable to see a healthcare professional for personalized advice.

In summary, while the brown discharge could be indicative of either pre-menstrual symptoms or implantation bleeding, the negative pregnancy test suggests that pregnancy is unlikely. Monitoring your cycle and symptoms will provide further clarity.
