Managing Side Effects of Roaccutane: Is It Safe to Continue? - Dermatology

I have been taking vitamin A acid for over two months?

I have been taking Roaccutane (isotretinoin) for two months, currently at a dosage of 10 mg every night.
My acne has improved, but I occasionally experience sudden redness and itching on my face.
This past week, I noticed an unusual increase in sexual desire during intercourse, accompanied by a complete body heat and sweating, but I am unable to achieve an erection.
Is this reversible? Should I stop the medication immediately? Note: I have been undergoing frequent blood tests, and my liver function and lipid levels are slightly elevated, which the doctor attributed to the medication.
However, the doctor advised to continue monitoring.
I have not yet followed up with the doctor and wanted to ask these questions first.
My height is 168 cm and my weight is 71 kg.

Dr. Wu Yuxin reply Dermatology

I haven't heard of such side effects.
If there are concerns, it is advisable to stop the medication and observe whether the abnormal conditions improve.

When managing side effects of Roaccutane (Isotretinoin), it is crucial to understand both the benefits and potential adverse effects of this medication. Roaccutane is widely used for treating severe acne, and while it can be highly effective, it also comes with a range of side effects that patients should be aware of.

From your description, it seems you have been on Roaccutane for two months and have noticed some improvements in your acne, but you are also experiencing redness and itching on your skin. These symptoms can be common side effects of Roaccutane, as the medication works by reducing oil production in the skin and promoting skin cell turnover, which can lead to dryness and irritation. It is essential to maintain a good skincare routine while on this medication, including the use of gentle cleansers and moisturizers to help manage dryness and irritation.

Regarding your concerns about increased libido and the inability to achieve an erection, these symptoms are less commonly discussed but can occur. Isotretinoin can affect hormonal levels and may lead to changes in sexual function for some individuals. The fact that you are experiencing heightened sexual desire but difficulty with erection could be a complex interplay of psychological and physiological factors. It is essential to note that many side effects of Roaccutane are reversible after discontinuation of the medication, but it is crucial to discuss these symptoms with your healthcare provider before making any decisions about stopping the medication.

You also mentioned that you have been experiencing elevated liver function tests and increased lipid levels. Roaccutane is known to potentially affect liver enzymes and lipid profiles, which is why regular monitoring through blood tests is necessary during treatment. If your liver function tests are only slightly elevated, your doctor may choose to continue monitoring rather than immediately discontinuing the medication. However, if there are significant elevations or if you experience symptoms such as jaundice, abdominal pain, or severe fatigue, it would be essential to contact your healthcare provider immediately.

In terms of whether you should stop taking Roaccutane, it is generally not advisable to make that decision without consulting your doctor. Your healthcare provider can assess the severity of your side effects, weigh the benefits of continuing treatment against the risks, and determine the best course of action. If the side effects are manageable and not severely impacting your quality of life, your doctor may recommend continuing the medication with close monitoring. However, if the side effects become intolerable or pose a significant health risk, your doctor may suggest discontinuation or adjusting the dosage.

In summary, while Roaccutane can be an effective treatment for severe acne, it is essential to manage its side effects carefully. Regular communication with your healthcare provider is crucial to ensure that any adverse effects are addressed promptly and appropriately. If you have concerns about your symptoms or the safety of continuing the medication, do not hesitate to reach out to your doctor for personalized advice and guidance.
